Hormone Replacement Therapy (HRT)#

What HRT Does#

Hormone therapy works by shifting the dominant hormone in your system—either introducing testosterone to suppress estrogen's effects, or introducing estrogen (often with testosterone blockers) to suppress testosterone's effects. The result is a gradual development of secondary sex characteristics associated with the dominant hormone: a second puberty, essentially, that unfolds over months and years.

The timeline matters. Unlike surgery, which produces immediate physical changes, HRT is slow. Voice deepening on testosterone typically begins within the first few months but continues developing for one to two years. Breast development on estrogen can take three to five years to reach full potential. Fat redistribution, muscle changes, body hair patterns—all of these shift gradually, often in ways you won't notice day-to-day but will recognize when you compare photos from six months apart.

I remember the first time someone who hadn't seen me in a year commented on how different I looked. I couldn't see it myself—the changes had happened so incrementally that they felt invisible. But looking back at photos from before I started T, the difference was undeniable. That's the strange gift of HRT's slowness: you get to live into the changes rather than having them thrust upon you all at once.

This gradualness has advantages beyond the psychological. It gives you time to assess how you feel about changes as they happen. Most HRT effects can be stopped or reversed (to varying degrees) if you change your mind early in the process. But some changes become permanent relatively quickly—voice deepening on testosterone, for instance, or breast development on estrogen. Understanding which changes are reversible and which aren't is essential for making informed decisions.

Masculinizing Hormone Therapy#

Testosterone is the primary hormone used for masculinization, and it's available in several forms with different administration schedules, absorption rates, and practical considerations.

Injectable testosterone, typically cypionate or enanthate, is the most common and often most cost-effective option. You inject it intramuscularly or subcutaneously, typically every one to two weeks, and after training from your provider, most people do this at home. The advantage is reliable absorption and flexibility in dosing. The disadvantage is that some people experience mood or energy fluctuations as hormone levels peak after injection and decline before the next dose. I've always done my injections weekly rather than biweekly—I found the emotional rollercoaster of every-two-weeks dosing exhausting, with a noticeable crash in the days before my next shot. Splitting the dose into smaller, more frequent injections smoothed things out considerably.

Topical testosterone, available as gels or creams under brand names like AndroGel and Testim or as generic compounded formulations, provides more stable daily hormone levels since you apply it every day. The main considerations are cost (topicals tend to be more expensive), the need to avoid skin contact with others until the gel dries completely, and the daily routine itself—some people find it grounding, others find it tedious. Testosterone patches offer another daily-application option with steady hormone release, though skin irritation at the application site is a common complaint.

For those who want to think about their HRT as infrequently as possible, long-acting injectable testosterone (undecanoate, brand name Aveed in the US) is administered every ten to twelve weeks by a healthcare provider after an initial loading phase. This approach requires clinic visits for each injection and is often more expensive, but for some people the trade-off is worth it.

Typical starting doses for people seeking full masculinization range from 50 to 100mg weekly of injectable testosterone, adjusted based on blood levels and how you feel (Hembree et al., 2017). For those seeking more subtle changes—often described as "low-dose" or "microdose" testosterone—doses might be 20 to 40mg weekly or equivalent. The goal with low-dose approaches is typically slower, more controlled changes that allow you to find a stopping point that feels right rather than pursuing maximum masculinization.

The first changes most people notice are increased energy, heightened libido, and clitoral growth—often within the first few weeks to months. Menstruation typically stops within two to six months, though this varies considerably between individuals. Voice deepening usually begins within three to twelve months and is among the permanent changes; once your vocal cords have thickened, stopping testosterone won't reverse that shift. Facial and body hair growth is gradual, developing over one to five years and influenced heavily by genetics—if the men in your family can't grow beards, testosterone won't override that blueprint. Fat redistribution away from hips and toward the abdomen happens over months to years, as does the increase in muscle mass that continues with ongoing testosterone use.

Feminizing Hormone Therapy#

Feminizing HRT typically combines estrogen with an anti-androgen (testosterone blocker), though some people use estrogen alone—particularly after orchiectomy, when testosterone production has been surgically eliminated.

Estradiol is the form of estrogen most commonly used, and it's available as pills (taken orally or dissolved under the tongue), patches, injections, and topical gels or creams. Oral estradiol is convenient and widely available, and taking it sublingually—letting it dissolve under the tongue—may reduce strain on the liver compared to swallowing (Unger, 2016). Typical doses range from 2 to 8mg daily, depending on goals and individual response.

Transdermal estradiol, delivered through patches or gels, provides steady hormone levels and bypasses the liver, which may reduce blood clot risk—an important consideration for anyone over 40 or with other clotting risk factors (T'Sjoen et al., 2019). Patches are typically changed once or twice weekly. Injectable estradiol (valerate or cypionate) is administered every one to two weeks and provides reliable absorption; some people find injections produce more stable levels and emotional equilibrium than oral administration.

Anti-androgens suppress testosterone production or block its effects. The most common in the US is spironolactone, a daily pill that also acts as a diuretic—meaning you'll urinate more and need to stay hydrated. Spironolactone requires monitoring potassium levels through regular blood work. Outside the US, cyproterone acetate is common; it's more potent but carries its own risks, including potential effects on mood and liver function. GnRH agonists are another option—very effective but typically more expensive.

After orchiectomy, many people find they no longer need anti-androgens since the primary testosterone source has been removed. Estrogen alone may be sufficient, often at lower doses than before surgery. This is one of the ways surgery can simplify long-term HRT management—fewer medications, fewer side effects, more straightforward maintenance.

Progesterone remains controversial in feminizing HRT. Some providers and patients report it enhances breast development, improves mood, and supports libido. Others find no benefit or experience negative effects. The research is limited and inconclusive (Coleman et al., 2022). If you're curious, it's worth discussing with your provider as something to potentially try and evaluate rather than dismissing outright.

The timeline for feminizing changes begins with skin softening, often within weeks. Breast development starts within the first few months as tenderness and early growth, continuing for several years—patience is essential here, as full development can take three to five years. Fat redistribution to hips and thighs develops over one to three years. Body hair becomes finer and sparser. Spontaneous erections decrease, often significantly. Many people report emotional changes—frequently described as a wider range of accessible feelings—in the first months of treatment.

Estrogen does not change bone structure or voice pitch. Voice training is often necessary for transfeminine people seeking to alter their vocal presentation—this is a skill that can be developed through practice, often with the help of a speech-language pathologist or voice coach.

Nonbinary and Customized Approaches#

The standard frameworks—"masculinizing" and "feminizing"—assume binary endpoints that don't reflect many people's goals. If you want some changes but not others, or want to pause at a particular point, or want a combination that doesn't fit either standard path, you're not alone. The WPATH Standards of Care explicitly recognize that gender-affirming care should be individualized based on each person's unique goals and needs (Coleman et al., 2022).

Low-dose or microdose regimens involve taking lower amounts of hormones to produce slower, subtler changes. This allows you to assess how you feel as changes develop and potentially stop at a point that feels right rather than pursuing full masculinization or feminization. Some people stay on low doses indefinitely; others use them as a starting point before deciding whether to increase.

Selective goals might mean wanting a deeper voice but minimal body hair, or wanting breast development but maintaining erectile function. Hormones don't work à la carte—you can't pick exactly which changes you'll get—but dose levels, duration, and timing all influence outcomes. Working with a knowledgeable provider to understand what's likely, what's possible, and what's permanent helps you make informed choices within the constraints of biology.

Mixed approaches are also possible. Some people cycle between hormones or use combinations that don't fit standard protocols. These paths often require providers who are flexible and willing to work with non-standard goals—not all are, which makes finding the right provider especially important for people whose goals don't fit neatly into binary frameworks.

I'm an example of a non-standard path. After nullification surgery removed my testes, I chose to continue testosterone rather than switching to estrogen or going without hormones. My primary dysphoria was anatomical—having a penis and testicles felt wrong—but I don't experience dysphoria about testosterone's effects on my body. I like how I feel on T: the muscle mass, the energy, the body hair. Continuing testosterone after nullification meant deliberately choosing to maintain that hormonal profile rather than defaulting to it because of organs I was born with. That distinction—between having a hormonal balance imposed by your anatomy versus choosing one that feels right—matters to me. It's the difference between circumstance and agency.

This isn't the right path for everyone, and it's not meant to be a template. It's one example of how surgical and hormonal decisions can be made independently based on what actually causes you dysphoria versus what doesn't. Your combination might look completely different, and that's exactly as it should be.

HRT and Surgical Planning#

If surgery is in your future, understanding how hormones interact with surgical requirements and recovery is essential planning.

Pre-surgical hormone requirements vary by procedure and surgeon. Some surgeons require 12 months of hormone therapy before genital surgery; others don't (Coleman et al., 2022). For metoidioplasty, longer testosterone use (often two or more years) maximizes clitoral growth before surgery—the more growth achieved hormonally, the more tissue surgeons have to work with. For orchiectomy or vaginoplasty, surgeons may have estrogen duration requirements. Always clarify requirements early in your surgical planning—discovering at the consultation that you need another year of HRT before you're eligible is frustrating and can derail carefully laid timelines.

Stopping hormones before surgery is often required, typically two to four weeks prior, to reduce blood clot risk during and after the procedure. This gap can feel difficult emotionally, and some people experience a temporary return of dysphoria during this time. When I stopped testosterone before my nulloplasty, I noticed myself getting more irritable and fatigued in that second week—nothing catastrophic, but enough that I was glad I'd warned my support people it might happen. Knowing the hormone pause is temporary and planning for it—maybe arranging extra support during those weeks, scheduling lighter responsibilities, being gentle with yourself—helps.

Post-surgical hormone needs depend on what organs remain. If you've had an orchiectomy, your body no longer produces significant testosterone, so you'll need some form of hormone replacement (either estrogen or testosterone, depending on your goals) for long-term health—particularly bone density (Iwamoto et al., 2019). If you've had a hysterectomy and oophorectomy, the same applies: you'll need ongoing hormones unless you want to manage the health effects of having very low hormone levels, which can include bone loss, cardiovascular changes, and cognitive effects over time. Your surgeon and endocrinologist should coordinate on post-surgical hormone management, though in my experience, you may need to facilitate that communication yourself rather than assuming it will happen automatically.

Monitoring and Safety#

HRT requires regular blood work, especially in the first year and after any dose changes. Typical monitoring includes hormone levels (testosterone, estradiol), hematocrit (red blood cell count, which testosterone can increase), liver function, lipid panel, and sometimes prolactin or other markers depending on your regimen (Hembree et al., 2017).

Common side effects on testosterone include acne, increased body odor, changes in mood or energy, and potential hair thinning or loss at the temples. The acne surprised me—I'd had clear skin as a teenager and somehow expected that to continue. Instead, I spent my first year on T learning skincare routines I'd never needed before. Hematocrit elevation is a more serious medical concern that requires monitoring—if levels get too high, you may need to adjust your dose or donate blood to bring red blood cell counts down.

Common side effects on estrogen include breast tenderness, mood changes, decreased libido, and potential nausea when starting. Blood clot risk is the most serious concern, particularly with oral estrogen in people over 40, smokers, or those with clotting disorders (T'Sjoen et al., 2019). Transdermal estrogen (patches, gel) appears to carry lower clot risk than oral forms, which is why many providers recommend non-oral routes for higher-risk individuals.

Contraindications—conditions that make HRT risky—include history of blood clots, certain hormone-sensitive cancers, and severe liver or cardiovascular disease. Having risk factors doesn't necessarily mean HRT is impossible, but it means careful evaluation and possibly modified approaches. A provider experienced in transgender care can help weigh risks against benefits for your specific situation.

The goal of monitoring isn't to catch you doing something wrong. It's to ensure your body is handling the hormones well and to adjust your regimen for optimal health and effectiveness. A good provider relationship involves honest communication about how you're feeling, what's working, and what isn't. If something feels off, saying so isn't complaining—it's giving your provider the information they need to help you.

Accessing HRT#

How you access HRT depends on your location, insurance, and the providers available to you.

Informed consent clinics provide HRT based on your stated identity and goals without requiring letters from therapists or extensive psychiatric evaluation. You receive information about risks, benefits, and alternatives; you confirm you understand; and treatment can begin. This model treats you as the expert on your own identity—which you are. Planned Parenthood locations in many areas, many LGBTQ+ health centers, and telehealth services like FOLX, Plume, and QueerDoc operate on informed consent models.

Traditional gatekeeping models require one or more letters from mental health providers before HRT can begin. While this model is becoming less common, some providers and insurance companies still require it. The WPATH Standards of Care have moved away from strict gatekeeping requirements, but not all practitioners have updated their practices accordingly (Coleman et al., 2022). If you're dealing with gatekeeping, having a therapist who understands gender-affirming care and can write letters efficiently helps—though it's reasonable to seek providers who don't require this.

Insurance coverage for HRT varies widely. Many insurance plans now cover hormone therapy for gender dysphoria, but you may need prior authorization, and some plans have exclusions. Generic testosterone and estradiol are relatively inexpensive even without insurance—often $20 to $50 per month depending on form and pharmacy. Brand-name medications and some delivery methods cost more.

DIY HRT—obtaining hormones without a prescription—is a reality for people who face barriers to legitimate care: inability to find knowledgeable providers, insurance exclusions, lack of money for appointments, or living in regions where access is restricted or dangerous. I'm not going to pretend this doesn't happen or that everyone who does it is being reckless. For some people, DIY is the only option that feels accessible or safe, and moralistic finger-wagging doesn't change material circumstances.

If you're considering or currently using non-prescribed hormones, harm reduction matters. Source quality is unpredictable—some online sources sell legitimate medications, others sell contaminated or misdosed products. Getting blood work done (which you can often order yourself through services like Walk-In Lab or Ulta Lab Tests) helps you monitor your levels and catch problems early. Using standard doses rather than assuming more is better reduces risk. Connecting with community resources and online groups where people share information about sourcing and safety can help—though information quality varies, so approach with appropriate skepticism.

The goal should still be getting into legitimate care when possible. Informed consent clinics and telehealth have made this more accessible than ever. But I understand that "when possible" isn't the same as "right now" for everyone, and I'm not interested in shaming people for making the best choices available to them in imperfect circumstances.

Long-Term Considerations#

HRT is often a long-term or lifelong commitment, and understanding what that means helps you plan.

Fertility preservation should be discussed before starting HRT if having biological children matters to you. Testosterone typically stops ovulation; estrogen typically stops sperm production. These effects may or may not be reversible if you stop hormones, and the research on long-term fertility after HRT is limited (Iwamoto et al., 2019). Banking eggs or sperm before starting hormones is the most reliable option if future biological children are important to you. Fertility addresses fertility preservation in detail.

Bone density requires adequate hormone levels. If you're on very low doses or if you've had gonads removed without adequate hormone replacement, you're at risk for osteoporosis over time. This is manageable with proper monitoring—regular DEXA scans to assess bone density—and ensuring your hormone levels stay in a healthy range. It's one of the reasons post-surgical hormone management matters so much.

Cardiovascular health is a long-term consideration, particularly with testosterone (which can affect cholesterol profiles) and with estrogen (clot risk, particularly with oral forms). Regular monitoring and attention to other cardiovascular risk factors—smoking, diet, exercise, blood pressure—matters for overall health, not just HRT safety.

What happens if you stop depends on what changes have occurred. Some effects reverse if you stop HRT relatively early: fat distribution shifts back, skin texture changes, menstruation returns or spontaneous erections return. Others are permanent: voice deepening on testosterone, breast growth on estrogen, and depending on duration, facial hair growth or male-pattern baldness. Understanding what's reversible helps you make decisions with appropriate weight—recognizing that some choices close doors while others remain open.

Finding the Right Provider#

A good HRT provider is knowledgeable about gender-affirming care, operates from an informed consent framework (or at least doesn't create unnecessary barriers), listens to your goals rather than assuming what you want, and is willing to work with non-standard protocols if that's what you need.

Questions worth asking at a first appointment: How do you approach dosing—do you work with patients to find what feels right, or do you follow strict protocols? What's your experience with nonbinary patients or patients seeking partial effects? How do you handle coordination with surgeons if I'm planning surgery? What does your monitoring schedule look like?

If a provider dismisses your goals, questions your identity, or seems unfamiliar with current standards of care, finding someone else is worth the effort. I've had providers who treated my non-standard path—nullification while continuing testosterone—as confusing or suspect, and providers who engaged with genuine curiosity about what I wanted and why. The difference in quality of care was profound. You deserve a partner in your healthcare, not a gatekeeper who thinks they know your body and your identity better than you do.
