The cited radial-forearm phalloplasty paper does not establish a one-millimeter-per-day nerve rule or a universal genital-surgery scar and sensation calendar. The operating team is the source for expected changes and reasons to contact them.

Documentation creates objective evidence of transformation when your eyes fail you.

When I was in my darkest post-operative days—swollen, uncomfortable, questioning everything—looking back at photos from the first week reminded me how far I'd come. The me of week one couldn't stand without assistance. The me of week six was walking to the corner store. The evidence was there, even when I couldn't feel it in my body. Research on health journaling supports this: structured documentation creates tangible markers of progress that combat the distorted perception that often accompanies healing (Pennebaker & Smyth, 2016).

Remembering where you started matters because healing happens incrementally, imperceptibly, until one day you realize you're doing something that was impossible two months ago. Combating dysphoria during healing is perhaps the most critical function of documentation for trans patients specifically. Healing bodies often don't look like the bodies we imagined. Swelling distorts. Bruising discolors. Surgical sites look raw and foreign before they settle into something recognizable. When dysphoria whispers that this was a mistake, that it looks wrong, that it will never look right—photographs from earlier weeks that show how much worse it was can quiet those voices with evidence.

The psychological weight of the "in-between" phase deserves acknowledgment. You've had the surgery. The transformation you wanted has technically happened. But what you see in the mirror doesn't yet match what you envisioned, because healing obscures results. This mismatch can trigger dysphoria even more intense than what you felt before surgery—because now you've done the irreversible thing, and what if it doesn't work? Documentation provides the antidote: visual proof that your body is moving in the right direction, even when it doesn't feel that way.

Fighting the "It's Not Working" Spiral#

Every surgical recovery involves a moment—usually somewhere between week three and week eight—when the initial adrenaline has worn off, the support system has returned to normal life, and you're alone with a body that doesn't yet look like you hoped it would. This is when the spiral hits.

Is this as good as it gets? Did something go wrong? Everyone else looks better by now. Maybe I made a mistake. Maybe the surgeon messed up. Maybe my body just doesn't heal right. Maybe I should have chosen a different procedure, a different surgeon, a different everything.

Documentation provides the antidote: objective markers that prove change is happening even when you can't perceive it.

I remember sitting on my bathroom floor in San Francisco, convinced my healing had stalled. The swelling looked exactly the same as it had for days. Nothing was changing. I was stuck. This was as good as it was going to get.

Then I pulled up photos from two weeks prior—the swelling that day had been so severe I'd barely been able to see the surgical site. Compared to what I was looking at in that moment, the difference was obvious. Dramatic, even. But I hadn't noticed it happening because I'd seen it every single day, and gradual change is invisible to daily observation.

Some useful objective markers to track include swelling circumference (if measurable for your procedure) with a soft tape measure, bruise progression through dated photos showing color changes from purple to green to yellow to resolved, scar appearance photographed monthly showing color, texture, and elevation changes, mobility achievements noted in writing such as "Today I walked to the elevator—yesterday I couldn't," pain levels on a 0-10 scale at consistent times each day, and sensation mapping noting where you feel tingling, numbness, or emerging sensitivity.

Emotional anchoring through documentation helps you remember what you actually felt at different stages. Reading a journal entry from week two that says "the pain is manageable and I'm grateful to be healing" can anchor you when week six feels intolerable. Memory is notoriously unreliable, especially memory filtered through physical discomfort and emotional volatility. Written records preserve what was true before your current state rewrote the narrative.

I've gone back and read my own recovery notes from those San Francisco hotel room days. Some of what I wrote surprises me—moments of joy and gratitude that I'd completely forgotten, alongside struggles I remember vividly. The documentation captured a more complete picture than my memory alone preserved.

Celebrating Milestones#

Recovery milestones deserve acknowledgment. They deserve to be documented, remembered, and yes—celebrated. Surgical recovery is hard. Every step forward matters.

First day without drains is a freedom most people take for granted until they've experienced the indignity and inconvenience of drain bulbs attached to your body. The tubes pulling at insertion sites. The careful management when getting dressed. The tracking of output amounts. When they come out, it's worth noting the date, how you felt, the relief of finally being able to move without managing plastic tubes.

First time standing unassisted marks significant healing for major bottom surgeries. The simple act of standing without support represents a real milestone. The first time you push yourself up from the bed and stay upright without someone's arm to steady you—that's worth remembering.

First shower is practically a religious experience after days or weeks of sponge baths, careful avoidance of water on surgical sites, and feeling generally unclean. Document it.

First successful dilation (for vaginoplasty patients) is both a clinical milestone and an emotional one—a moment of taking agency in your own recovery, of engaging with your body in a new way.

First time wearing clothes comfortably marks when pants or underwear stopped causing discomfort, when you could sit without a donut pillow, when clothing just fit the way it's supposed to.

First time you went outside after being confined to a hospital room, then a hotel room or bedroom, marks a turning point in recovery when you finally step outside and feel sun and air on your skin.

Documenting joy is as important as documenting complications. The first time you looked at your body and felt euphoria instead of neutrality. The moment you realized you weren't flinching at mirrors anymore. The morning you woke up and just felt right in a way you hadn't before. These moments deserve preservation.

I have a photo from approximately week three of my recovery—not a clinical photo, but a selfie of my face. I'm smiling. Actually smiling. The caption I wrote at the time was something like "I did it." Looking at that photo now, almost two years later, I can still access the feeling of that moment: the wonder that I had actually gone through with something I'd wanted for decades.

Create rituals around milestones if that helps you. Some people light candles. Some write letters to their future selves. Some buy small gifts—jewelry, art, meaningful objects that will remind them of this period. Some simply sit quietly and acknowledge what they've accomplished. There's no wrong way to celebrate. The point is recognition: this is hard, and you're doing it.

Creating Your Personal Archive#

Beyond day-to-day documentation, consider what you want your longer-term archive to contain. This is the material you might look back at in five years, ten years, a lifetime from now.

Organizing your documentation matters more than you think it will. In the acute post-operative phase, you'll be tired, probably medicated, and not inclined toward meticulous file management. But future you will be grateful for whatever structure you establish early. Create folders by date or week. Name files consistently. Separate clinical photos from personal reflections from celebratory images.

A folder structure that works might include a main Surgery_Documentation folder containing subfolders for Clinical_Photos (with weekly subfolders like Week_01, Week_02, Week_03), Personal_Photos (which includes face and identifying info), Journal_Entries, Medical_Records (containing Discharge_Instructions, Follow_Up_Notes, and Lab_Results), and Symptom_Tracking.

Naming conventions help too. Something like 2024-08-15_Day07_Clinical_Front.jpg tells you immediately when the photo was taken, how many days post-op, what type of photo, and what angle. Compare that to IMG_4392.jpg—meaningless without opening the file.

Storing safely means redundancy—more than one location, more than one device—and security, which we'll address in detail later. Losing documentation to a phone crash or cloud service change is devastating in ways you can't appreciate until it happens.

Make backups. Multiple backups. At minimum: one local (external hard drive or your computer's drive), one cloud (encrypted), and ideally one additional location (another external drive kept somewhere separate). Yes, this seems excessive. No, it won't feel excessive when your phone dies and your cloud account gets locked and you realize all your recovery documentation is gone.

Reflecting on your journey is what this archive ultimately enables. Transition is a process, not an event. Surgery is a guide, not the whole story. Years from now, having a comprehensive record of this transformative period of your life creates opportunities for meaning-making that don't exist without documentation.

I periodically revisit my recovery documentation—not obsessively, but deliberately. Sometimes on anniversaries: the one-year mark of surgery, the two-year mark. Sometimes when I'm struggling and need to remember how far I've come. Sometimes when I'm helping someone else prepare for their own surgery and want to share what early recovery actually looked like. The archive serves many purposes across time.


For Your Doctors: Clinical Documentation#

Medical Photography Guidelines#

If your surgical team asks for photos, ask which views, scale reference, lighting, privacy protections, file-sharing method, and timing they want for your operation. Taking a comparable view when practical can make your own sequence easier to discuss, but the cited Rennert and WOCN entries do not establish a bottom-surgery two-distance protocol, exact camera distances, or a universal clinical photography standard.

Specific technical requirements include: including a paper ruler with centimeter markings placed 1-2 inches from the skin (this provides scale that lets surgeons assess actual wound size and any changes over time—without scale reference, photos can be misleading); using a plain, non-reflective background in neutral colors (a white towel or sheet works well—avoid patterned backgrounds, reflective surfaces, or backgrounds that compete visually with the surgical site); using natural daylight or diffused artificial light (avoid direct flash, which causes glare and washes out tissue color differences that indicate healing or complications, and avoid direct sunlight, which creates harsh shadows that obscure detail); and using the same camera/phone settings each time (avoid zooming digitally—physically move closer or farther away instead, keep HDR and filters off, and use the same device if possible).

What your surgeon specifically wants to see includes wound bed color and tissue type (healthy healing tissue is pink/red; yellow or green tissue, dark discoloration, or white patches need attention), wound edges (noting whether they're flat, rolled, or separating—edges that are pulling apart or rolling inward indicate problems), any drainage characteristics (clear or light pink drainage is typically normal; thick, colored, or foul-smelling drainage warrants immediate contact with your surgical team), and the extent of redness spreading beyond the wound margin (some redness immediately adjacent to a surgical site is normal; redness extending outward—especially redness that's spreading over time—can indicate infection; measuring from the wound edge matters—redness extending more than five centimeters is concerning and warrants immediate communication with your surgical team) (Wound, Ostomy and Continence Nurses Society, 2016).

For bottom surgery specifically, document suture line integrity (are sutures in place? any areas where they've pulled through or separated?), swelling progression (compare to previous photos at same angle), any areas of wound separation (even small openings should be documented), and tissue color (particularly any areas turning dark, gray, or black—this requires immediate medical attention).

Signs of complications requiring immediate photos and surgeon contact include spreading redness (especially if warm to touch), increased or discolored discharge (yellow, green, brown), wound opening or separation, tissue color changes toward black or gray (sign of tissue death), fever combined with wound changes, sudden pain increases not explained by activity, and foul odor from the surgical site.

When you notice any of these, photograph first, then contact your surgical team. Having visual documentation ready when you call enables faster and more accurate triage.

When I developed sepsis during my revision surgery recovery in November 2025, the photos I'd taken in the days leading up to the crisis helped the medical team understand what had changed and how quickly. "It started looking like this on Tuesday" is less useful than "here's a photo from Tuesday at 4pm, and here's another from Wednesday morning." Noticing that something felt wrong—the pain worsening instead of improving, the unusual fatigue, the temperature that climbed to 103 degrees—and having documentation of what the surgical site had looked like before the change helped the emergency department understand the trajectory of my infection. If I hadn't been paying attention to my body's signals and documenting what I saw, things could have ended differently.

What to Document and When#

Timing protocols vary by recovery phase.

Days 1-7 (Acute Phase): Daily photos capture the period of highest complication risk. Major gender-affirming surgery centers including the surgical practice and OHSU recommend photographing daily during this critical window. This feels like a lot, especially when you're exhausted and in pain. Do it anyway. These photos are most valuable if something goes wrong—they'll show your surgical team exactly how things progressed.

Weeks 2-6 (Early Healing): Weekly documentation as the acute phase transitions to early remodeling. Swelling is still resolving, sutures may still be in place or recently removed, and the surgical site is still actively changing. Weekly photos capture this transformation.

Months 2-6: Twice-monthly photos track the intermediate healing phase. Changes are slower but still occurring. This is when many people stop documenting consistently—don't. The comparison value is highest when you have regular intervals over the full healing period.

Months 6-18: Monthly photos track the long remodeling phase, which can continue for twelve months or longer for bottom surgery (Monstrey et al., 2009). Scar maturation, final swelling resolution, and tissue settling happen during this extended period.

Beyond scheduled intervals, photograph before every follow-up appointment (giving your surgeon current visual data to discuss), at any significant change in wound appearance (don't wait for your scheduled photo if something looks different), before and after dilation sessions (for vaginoplasty patients, this tracks progress and helps identify issues), and whenever you're concerned about something—a photo you send to your surgeon is worth a thousand worried words.

Healing milestones worth documenting include drain removal, suture removal, catheter removal, first signs of nerve sensation returning, visible reduction in swelling, scar color changes over time, first time wearing underwear/pants comfortably, and full sitting tolerance achieved.

Complications require immediate photographic documentation. If something looks wrong, photograph it before you contact your surgical team—this gives them visual data to assess the urgency. Major complications may progress over hours; having a sequence of timestamped images shows rate of change, which is clinically significant.

Sensation return deserves tracking even though you can't photograph it. Keep written notes about where you're feeling tingling, temperature sensitivity (can you feel hot versus cold water?), the "electrical zap" sensations that indicate nerve regeneration, progressive return of touch sensation across different areas, any areas of hypersensitivity (pain from light touch), and any areas of persistent numbness.

Notes about sensation, discomfort, or change can help you describe what you noticed at a follow-up. They cannot establish a nerve-regeneration rate or forecast; ask the operating team which sensory changes matter for your operation and when they want contact.

I'm still dealing with uncomfortable electric-like sensations from nerve healing since my revision surgery. The calcifications that required revision had sharp spiky edges that were damaging nerve tissue, and tracking those sensations over time has been essential for understanding my healing trajectory and communicating with my medical team about what's normal versus what might need attention.

Scar appearance evolves dramatically over twelve to eighteen months. Monthly photos capture this progression, which is nearly impossible to perceive otherwise. Scars typically look worst around 3-6 months (red, raised, firm) before gradually softening and fading. Without photos from the earlier periods, you won't appreciate how much improvement occurred.

Tracking Logs: What to Record Daily#

During the first two weeks especially, keep a daily tracking log that includes physical measurements (temperature twice daily minimum—morning and evening, pain level on a 0-10 scale at consistent times, drain output amounts and colors if applicable, and any measurements your surgical team requested), medications (what you took, what time you took it, effectiveness—did the pain medication actually work?—and any side effects), functional progress (mobility achievements like "walked to bathroom unassisted" or "walked to end of hallway," activities of daily living like "first shower" or "dressed myself," bowel movements including frequency and difficulty since constipation is common post-surgery, food and fluid intake, and sleep quality and duration), and observations (wound appearance notes beyond photos, unusual symptoms or sensations, questions for your medical team, and concerns even minor ones).

KJ kept a spreadsheet during my acute recovery. Temperature, pain level, medications, drain output, notes. Every four hours during the worst of it. It felt excessive at the time—who needs this level of detail? We needed it exactly once, when we had to call the surgical team about a potential problem. Being able to say "temperature was 99.1 at 6am, 99.8 at 2pm, 100.4 at 8pm—so it's been rising steadily" gave them information they couldn't have gotten from "I think I might have a fever." That one call justified all the tracking.

Storage and Privacy for Medical Documentation#

Intimate medical photographs demand robust protection. Your surgical documentation contains deeply personal information that, if compromised, could be weaponized against you. Trans people face documented harassment risks—45% of transgender respondents in Anti-Defamation League research experienced severe online harassment including doxxing in the past year (Anti-Defamation League, 2021). Medical information and body images are particularly dangerous in the wrong hands.

Secure storage means encryption you control. Zero-knowledge end-to-end encrypted cloud services ensure that even the provider cannot access your files. Options include Ente Photos (zero-knowledge encryption, open-source, cross-platform, independently audited—this is currently my recommendation for most users), Cryptee (zero-knowledge encryption for documents and photos, based in Estonia with strong privacy laws), and Proton Drive (Swiss privacy protections, password-protected sharing links that expire automatically, part of the Proton ecosystem which is good if you already use ProtonMail).

For local storage, VeraCrypt creates encrypted containers using AES-256 encryption (the U.S. government standard for classified information). These containers appear as virtual drives when mounted and remain completely inaccessible otherwise. VeraCrypt's hidden volume feature allows creating a decoy vault with a different password—providing plausible deniability if ever coerced to unlock the container (National Institute of Standards and Technology, 2001).

Standard cloud services—iCloud, Google Photos, Dropbox, OneDrive—are not sufficient for sensitive medical documentation. These services can be subpoenaed, breached, or accessed by company employees. 45% of data breaches are cloud-based (IBM Security, 2023). Major platforms including iCloud have experienced breaches exposing private photos.

HIPAA considerations apply to how you share with providers. Patient portal systems typically have HIPAA protection. Photos sent through your surgical center's official portal become part of your medical record with legal confidentiality requirements. Email and text to providers—even to your surgeon directly—may lack HIPAA protection depending on the communication method. These communications could potentially be accessed, leaked, or subpoenaed. Ask your surgical team about their preferred secure communication method. Many now use secure messaging apps or patient portal photo uploads specifically for post-operative documentation.

Who should see your documentation is entirely your decision. This includes romantic partners (even serious ones—you decide what medical information to share with partners), family members (even supportive ones), close friends, and future medical providers (you choose what goes into records that follow you). Create clear boundaries for yourself about what you're willing to share and with whom. It's okay for these boundaries to differ across relationships—you might share detailed documentation with your partner but only general updates with family.

Sharing with Your Medical Team Effectively#

Communication with your surgical team is where clinical documentation proves its value. Being organized and specific makes these communications more productive for everyone.

What to share includes photos of concerning changes (with date/time noted), photos before appointments (so your surgeon has current visual data), written symptom logs showing patterns over time, specific questions prompted by what you've documented, and any measurements your surgical team requested.

How to share: Use your patient portal when possible—this is typically the most secure option and ensures documentation becomes part of your medical record. If texting or emailing photos, strip metadata first (more on this below), use secure messaging apps when available, and include the date and time the photo was taken, your current symptoms, and any relevant context. Be specific—"The right side of the incision has increased redness spreading approximately 2 centimeters beyond the wound margin since yesterday" is far more useful than "it looks worse." Include comparison—"Attached are two photos—yesterday at 6pm and today at 10am—showing the change I'm concerned about."

Surgeons see many patients. Your documentation helps them track your specific trajectory without relying solely on clinical notes from weeks ago. Patients who bring organized photographic timelines to follow-up appointments enable more productive conversations—you can point to specific dates, show progression, and ask targeted questions.

Before appointments, review your documentation and identify what questions you want answered, what changes you want to discuss, and what photos illustrate your concerns or progress. Print key photos or have them easily accessible on your phone. Don't make your surgeon scroll through hundreds of images looking for the relevant ones.


Photography Best Practices: A Technical Guide#

Setting Up Your Photo Station#

Before surgery, establish a dedicated photo location with consistent conditions. This might be a bathroom, bedroom, or any space where you can control lighting and return to the exact same setup repeatedly.

For lighting, north-facing windows (in the northern hemisphere) provide consistent, diffused natural light that doesn't change dramatically throughout the day. Overcast days offer excellent lighting—bright but diffused, no harsh shadows. If using artificial light, use a ring light or diffused lamp positioned to illuminate evenly without casting harsh shadows; avoid overhead lighting alone, which creates shadows that obscure detail. Avoid direct sunlight, which creates harsh contrast and washes out subtle color differences. Avoid flash, which creates glare on skin and flattens the appearance of wounds.

For position markers, mark your standing position with tape on the floor and return to this exact spot each time. Mark the camera/phone position if using a tripod or mount; if someone else is taking photos, mark where they should stand. Note the time of day you take photos and try to be consistent—morning light differs from evening light, and swelling often differs at different times of day.

For background, a plain white, gray, or cream sheet or towel provides a neutral background that doesn't distract from the surgical site. Avoid patterned backgrounds, reflective surfaces, or cluttered environments. The background should be visible in the frame so photos look consistent.

For equipment, a tripod or phone mount significantly improves consistency; you can get phone tripods for under $20. A basic paper ruler with centimeter markings provides scale reference. A small ring light helps if natural lighting isn't reliable (also inexpensive—under $15 for basic models). A Bluetooth shutter remote makes positioning much easier if taking photos yourself.

Consistency for Comparison#

The entire point of serial photography is comparison over time. Inconsistency defeats that purpose.

Variables to control include the same location (which eliminates environmental variables—choose one spot and use it exclusively), same time of day (because swelling fluctuates—many people are more swollen in the morning while others swell more after activity, so pick a time and stick with it; if you must photograph at a different time, note it), same body position (meaning identical positioning each time—standing, lying, sitting; if standing, same posture and same weight distribution; if lying, same surface and same angle of recline), same camera distance (which requires either measuring or using visual landmarks—position yourself so the surgical site fills the same proportion of the frame each time; a tripod at a fixed position helps immensely), same framing (meaning the same view of the same area—if your week-one photo shows your surgical site centered in the frame, your week-six photo should match), and same camera settings (don't zoom digitally between sessions, don't switch between cameras with different lenses, keep HDR and filters off, and use the same device if possible).

Why this matters: Two photos taken at different distances, angles, and lighting can make identical healing look dramatically different—or make real change invisible. I've seen people panic because a poorly-lit photo made their surgical site look worse, and I've seen people miss concerning changes because favorable lighting hid them.

A practical approach: Take your first photo session immediately before surgery (or as early in recovery as you're able). This establishes your baseline and your setup. Document your setup—what position you stood in, where the camera was, what lighting you used, what time of day. Reference that documentation every subsequent session and replicate it as precisely as possible. Review photos immediately after taking them. If they don't match previous sessions, retake them.

Clinical Photos vs. Celebratory Photos#

These serve different purposes and follow different rules. Keep them separate.

Clinical photos prioritize accuracy for medical assessment. They use a neutral background, consistent lighting, a ruler for scale, no filters, no editing, full visibility of the area in question, and no identifying features (no face, no distinctive tattoos if possible). These photos are data.

Celebratory photos mark milestones and capture emotions. They may include your face, your expression, context about where you are. They can be artistic, filtered, shot in flattering light. They include whatever you want to remember about the moment. These photos are memories.

When to take each: clinical photos on your established schedule (daily first week, weekly after that, etc.) and celebratory photos whenever joy strikes—when you catch your reflection and smile, when you achieve something you couldn't before, when you want to remember this moment.

Managing dual purposes: Keep these in separate folders. Clinical photos need to remain unmodified and accessible for medical use. Celebratory photos are yours to edit, share, or keep private as you choose.

If you want to share progress with friends or community while protecting your privacy, you might share celebratory photos (with face) while keeping clinical photos (without face but showing surgical details) separate. But once they're mixed together in one folder, sorting becomes tedious and error-prone.

Privacy and Security for Photos#

Digital photography creates data that extends beyond the visible image. Before taking any sensitive photos, establish your privacy infrastructure.

EXIF metadata is embedded in every digital photo and may include GPS coordinates (exactly where the photo was taken), date and time (down to the second), device information (phone model, camera model), and in some cases your name or Apple ID.

If you share a photo without stripping this metadata, you're potentially sharing your home address, the device you own, and exactly when you were there. This information creates serious privacy and safety risks for trans people (Friedland & Sommer, 2010).

To strip metadata on desktop (recommended for thoroughness), you can use ExifCleaner (free, open-source, works on Windows/Mac/Linux—drag and drop files and metadata is removed) or ExifTool (command line—the most thorough option; the command exiftool -all= filename.jpg strips everything). On mobile, you can use Metapho (iOS) to view and remove metadata from photos on your phone, or Scrambled Exif (Android) to strip metadata before sharing.

Process every photo before sharing. Make this automatic: before any photo leaves your encrypted storage to be sent anywhere, it goes through metadata removal.

Disabling automatic photo backup is the most critical step many overlook. Before taking any sensitive images, check and disable automatic sync on iCloud Photos (Settings > Your Name > iCloud > Photos), Google Photos (backup & sync setting in the app), OneDrive (camera upload setting), Dropbox (camera upload setting), and any other cloud service with automatic backup.

These services may sync photos to their servers before you have a chance to process them. Your unstripped metadata goes with them. Your intimate medical photos end up on corporate servers you don't control.

The "Recently Deleted" folder on iOS retains copies for thirty days even after you delete the originals. On Android, recently deleted photos may persist in Google Photos trash. Account for this when managing sensitive images.

For maximum protection, consider using a device not connected to cloud accounts specifically for medical photography—an old phone with WiFi disabled, or a basic digital camera. If using your primary phone, disable cloud backup entirely before taking photos, then re-enable only after sensitive photos have been moved to encrypted storage and deleted from the camera roll (including recently deleted).

For shared device considerations, if others have access to your phone or computer (partners, family members, children), use a separate photo storage location with its own security. Don't rely on hiding files in obscure folders—use encryption. A password-protected album is better than nothing; an encrypted container (VeraCrypt) or encrypted cloud service (Ente) is better.


Journaling Your Journey#

The Evidence for Expressive Writing#

Research from the Pennebaker paradigm—the foundational body of work on expressive writing and health—demonstrates that writing about significant experiences for fifteen to twenty minutes across three to five sessions produces substantial health improvements (Pennebaker & Smyth, 2016). A meta-analysis found an effect size of d=0.47 (comparable to many established psychological interventions), including fewer stress-related doctor visits, improved immune function, faster wound healing, reduced blood pressure, and better mood and psychological wellbeing.

The mechanism seems to involve translating inchoate emotional experience into organized verbal narrative. The process of putting overwhelming feelings into words makes them more manageable, less overwhelming, more integrated into your sense of self and life story.

For surgical recovery specifically, the evidence supports combining structured symptom tracking with freeform expressive writing. Both serve important but different functions.

Structured Symptom Tracking#

The structured component creates a clinical record that supplements your photographs. This is data collection—systematic, consistent, factual.

Daily tracking should include physical metrics (pain levels on a 0-10 scale at consistent times, pain location and character—sharp? dull? throbbing? burning?—medications taken with dosages, times, and effectiveness, temperature morning and evening minimum, drain output amounts and colors if applicable, bowel movements since constipation is common post-surgery and worth tracking, and sleep quality and duration), wound observations (appearance notes beyond what photos capture, any changes since last observation, drainage characteristics, odor since foul smell is concerning, and surrounding skin condition), functional progress (mobility achievements, activities you were able to do, activities you couldn't yet do, and energy levels), and dilation tracking for vaginoplasty patients (duration, dilator size, difficulty level, any bleeding or discomfort, and notes on technique).

If you choose to track sensation, record what you notice in your own words without treating tingling, temperature change, or an electrical sensation as proof of a particular nerve process. Bring a new, worsening, absent, or concerning sensory change to the operating team; they can interpret it in the context of the operation and examination.

This sensation map becomes valuable at follow-up appointments months later when your surgeon asks about nerve recovery. Without documentation, you'll struggle to remember the progression.

Freeform Expressive Writing#

The expressive writing component processes the psychological dimensions of transformation. This isn't data collection—it's emotional processing.

Journaling can be a private space to process a profound change, but it is not a required response to surgery and does not need an outcome claim to be worthwhile. The cited Tordoff study concerns gender-affirming care in youth, not causal percentage reductions after surgery or the effect of documentation. A separate 2015 U.S. Transgender Survey analysis found adjusted associations after reported surgery; it also does not establish causation or a journaling intervention effect (Almazan & Keuroghlian, 2021).

What to write about includes your emotional responses (how you felt seeing surgical results for the first time, moments of gender euphoria and what triggered them, moments of dysphoria during healing and what triggered them, how your relationship to your body is changing, and grief that may surface unexpectedly—for lost time, for the journey's difficulty, for things you can't undo), your changing self-perception (how you see yourself in mirrors now, how you feel getting dressed, changes in how you move through the world, and integration of your new body into your sense of self), your relationships (how others' reactions affect you, conversations or avoided conversations about your surgery, changes in how you relate to partners, friends, and family, and who has been supportive and how), and processing difficulty (days when recovery feels impossible, fears about outcomes, anxiety about complications, moments of regret and what they're really about, and how you're coping or not).

The standard instructions for expressive writing are to write about your deepest thoughts and feelings, don't worry about spelling, grammar, or structure, write for yourself and not an audience, aim for 15-20 minutes at a time, and remember that you don't have to share this with anyone ever.

This isn't about producing polished prose. It's about processing experience through language. The benefit comes from the act of writing, not from anyone reading it.

When I lay in that San Francisco hospital bed with sepsis, missing my five-year anniversary with KJ, thousands of miles from my husband and our senior emotional support dog, I felt more alone than I'd ever felt in my life. I ghosted my online community. I didn't come close to sharing with them the gravity of how emotionally distressed I was. In my darkest moments, I didn't want to be alive anymore. Writing about that later—not in the moment, when I couldn't—became part of how I processed what had happened. Putting the experience into words helped me understand it, contain it, and eventually integrate it into my story without being destroyed by it.

Frequency and Format#

During acute recovery (weeks 1-2), keep multiple brief symptom entries daily (tracking log format) and do expressive writing as energy permits—even a few sentences counts.

During early healing (weeks 3-8), maintain comprehensive daily tracking entries and three to four expressive writing sessions of fifteen to twenty minutes each per week.

After two months, daily tracking can become abbreviated or move to every-other-day, with weekly reflections continuing to mark progress and process experience.

Research shows that patients log most consistently in the first fifteen days, then adherence drops (Smyth et al., 1999). Building the habit early and simplifying tracking as recovery progresses helps maintain useful documentation.

Format options include paper journal (no digital security concerns, can't be hacked, but can be lost or found by others), encrypted digital notes (Standard Notes, Cryptee, or notes stored in your encrypted container), voice memos (useful when you're too tired to type and can be transcribed later), and a hybrid approach (structured tracking in a spreadsheet, expressive writing in a journal).

Choose whatever format you'll actually use. The best system is one you maintain.

Recording Questions for Doctors#

Recovery generates countless questions—and you will not remember them all when your appointment arrives. Pain medications affect memory. Sleep deprivation affects memory. Stress affects memory.

Keep a running question list. As questions occur to you during recovery, write them down immediately. Don't trust yourself to remember later. Keep this list accessible—a notes app on your phone, a dedicated section of your journal, a document in your tracking folder.

Categorize if helpful: urgent (need to ask before next scheduled appointment), for next appointment, and general curiosity (when there's time).

Before appointments, review your question list, prioritize what matters most (appointment time is limited), bring the written list with you, and write down the answers—you won't remember everything your surgeon says.

After appointments, note what your surgical team said so you can refer back to it, such as "Dr. [Name] said to continue current protocol for two more weeks and return if X happens." This keeps you on track and prevents misremembering instructions.


Where Trans Surgery Communities Exist#

Online communities provide peer perspectives unavailable from medical sources—real experiences from real people who've been through what you're considering. Academic researchers have studied these communities because they capture information that clinical studies don't (Reeves et al., 2022).

Reddit hosts the largest surgery-specific discussions. r/Transgender_Surgeries (106,000+ members) is the most active community for surgery results, surgeon reviews, complications, and recovery experiences with people at every stage participating—researching, preparing, recovering, years post-op. r/phallo covers phalloplasty specifically, r/Metoidioplasty covers metoidioplasty specifically, r/ftm (~270,000 members) is a general transmasculine community that includes surgery discussion, and r/MtF (~470,000 members) is a general transfeminine community that includes surgery discussion.

Research from Cambridge University found that transgender individuals cite Reddit as their most popular platform for health information (Reeves et al., 2022). Academic analysis documents four types of support these communities provide: appraisal support (mutual understanding through shared identity), informational support (surgeon reviews, procedure details, recovery timelines), emotional support (validation during vulnerable periods), and network support (connecting people with resources and caregivers).

TransBucket (transbucket.com) maintains the most comprehensive photo repository for trans surgery results, showing a broader range of outcomes than curated surgeon portfolios. Unlike official before-and-after photos—which show favorable results with optimal lighting—TransBucket includes complications, suboptimal results, the messy middle phases of healing, and outcomes that surgeons would never include in marketing materials. Registration is required for access, which creates some privacy protection. TransBucket also has an associated Discord server for real-time discussion.

FTMSurgery.net and Phallo.net provide comprehensive procedure guides, surgeon directories, and patient reviews. Important disclosure: featured surgeons on these sites are paying advertisers, which doesn't invalidate the information but does mean it's not neutral. Treat featured listings as advertising; evaluate unpaid content more seriously.

Discord servers offer real-time support with significant privacy advantages that public forums can't provide. Research published in the Bulletin of Applied Transgender Studies found that Discord's invitation-only structure creates safer spaces for sensitive medical discussions—members can speak more freely when they're not worried about screenshots, search engine indexing, or random hostile visitors (Adams & Puckett, 2024). Finding surgery-specific Discord servers: squeee.club maintains a directory, and many are shared through Reddit communities via invitation links. Some surgeons' patient communities have their own Discord servers.

Facebook groups exist for specific surgeons and procedures but typically require membership approval for safety. Quality varies significantly.

The Benefits of Community Connection#

The community fills critical gaps that medical systems often don't address.

Information unavailable in medical literature covers much of what patients want to know—how recovery actually feels, what results look like at different timepoints, how to navigate specific complications, what questions to ask surgeons—that isn't covered in peer-reviewed papers. Community knowledge fills these gaps.

24/7 access during recovery means that when you're lying awake at 3 AM wondering if something is wrong, medical offices are closed but online communities aren't. Someone who's been through what you're experiencing can often provide reassurance or advice to help you make it to morning.

Connection with geographically distant peers matters because most trans people don't know anyone locally who's had their specific procedure. Online communities connect you with people across the world who share your experience.

For people of color, communities are particularly important. Research shows trans individuals specifically seek results from others with similar skin tones due to differences in scarring, healing, and aesthetic outcomes (Reeves et al., 2022). When surgeon portfolios primarily show light-skinned patients, community photo sharing provides crucial missing information.

Emotional validation during stigmatized medical experiences comes from hearing from others who've felt what you're feeling—the fear, the excitement, the doubt, the relief—and normalizes experiences that can feel isolating.

I found my community before my surgery—people who'd already had nulloplasty, people who were preparing for it, people considering it. That community gave me information I couldn't get from medical sources, answered questions I was embarrassed to ask doctors, and reminded me I wasn't alone in wanting something as unusual as genital nullification. After surgery, those same people celebrated my milestones and supported me through complications. The value was immeasurable.

The Risks of Community Engagement#

However, the psychological risks of viewing others' surgical results require active management.

Social comparison theory explains how viewing idealized images compels negative self-comparisons (Festinger, 1954). Research in the Journal of Technology in Behavioral Science found that passive social media engagement correlates positively with cosmetic surgery consideration through comparison-driven dissatisfaction (Dhir et al., 2021).

What this looks like in practice: You're two months post-op and your results look a certain way. You see someone else post their two-month results and theirs look "better." Now you're upset. You're convinced something went wrong with your surgery. You spend hours comparing, zooming in on photos, spiraling.

What you may not realize: That person photographed themselves in flattering light at the best moment of a good day. Their anatomy was different from yours. Their surgeon used a different technique. Their genetics produced different healing. The comparison is meaningless—but it doesn't feel meaningless when you're in it.

Surgical results shared online often represent optimal outcomes. Favorable lighting, favorable angles, favorable moments. The messy middle phases of healing are underrepresented because people don't post those as often. Limited representation of diverse body types, ages, and anatomies intensifies comparison for those who don't see themselves reflected.

Privacy risks for transgender individuals online are severe and documented. Anti-Defamation League research found that 45% of transgender respondents experienced severe online harassment including doxxing in the past year—the highest of any demographic group (Anti-Defamation League, 2021).

Transphobic harassment tactics specifically weaponize pre-transition photos, medical information, surgical status, legal names, and addresses and workplaces.

Content shared online can never be fully retracted. Screenshots persist after deletion. Organized harassment campaigns have targeted trans individuals using publicly shared information. Platforms like Instagram and TikTok are public by default with algorithm-driven distribution that makes reach unpredictable—you may intend to share with a small audience and have content spread far beyond your control.

Choosing Your Level of Visibility#

The first decision is how visible you want to be. There's no universally correct answer.

Public sharing means anyone can see your content. It offers maximum community reach and potential to help others, but also maximum exposure and risk. Algorithm-driven distribution means unpredictable audience. Screenshots and reposts are possible. Content may be found by employers, family, anyone.

Private or selected sharing limits your audience to approved followers only, private communities (Discord servers, closed Facebook groups), and offers reduced risk while maintaining community connection. It's still not completely secure—approved people can still screenshot.

Full privacy means not sharing surgical documentation online at all. It offers maximum protection, no risk of exposure, and is still possible to receive support in other ways (therapy, in-person relationships, private communication with individuals). This is a completely valid choice.

Your choice can change over time. Many people share freely in early recovery when they're seeking support, then pull back as they settle into post-surgical life. Others process privately at first, then contribute to community resources later. You're not locked into whatever you decide initially.

Protective Sharing Practices#

If you choose to participate in online communities, protective practices reduce risk.

Use pseudonymous accounts completely separate from your real identity—different username, different email, different photos. Never connect your surgical documentation account to your legal name.

Remove identifying features from photos including face (obviously), distinctive tattoos, unique jewelry, room backgrounds that reveal location, anything visible through windows, and mail, documents, or screens showing personal information.

Strip metadata from every photo before sharing (see digital security section above).

Disable location tagging on your camera and social media apps.

Start in private communities before considering any public sharing—Discord servers with verification processes, private subreddits or subreddit communities with approval requirements, and invite-only groups.

Recognize permanence. Share only what you're permanently comfortable being public, because that's the realistic expectation. As one TransBucket contributor reflected: "The internet is forever and I can't take it all back, even if my feelings on my online privacy have changed" (TransBucket, n.d.).

Consider waiting. You don't need to share while you're still in the emotional upheaval of early recovery. Many people wait months or years before contributing documentation. One community member noted waiting five years before feeling ready to share.

Handling Negative Engagement#

If you share publicly, you will encounter some negative engagement. Having a plan helps.

Block and move on. Engaging with trolls rewards their behavior. Don't argue, don't explain, don't try to educate. Block, report if the platform allows it, and don't give them more of your attention.

Not engaging is hard when someone attacks something deeply personal. The impulse to defend yourself, correct misinformation, or tell them off is strong. Research consistently shows that engagement prolongs and intensifies harassment (Jhaver et al., 2018). The best response is no response beyond blocking.

Protect your mental health. If negative comments are affecting you, reduce your visibility (make accounts private), close comments on posts, take breaks from platforms, mute notifications, and set boundaries on when you check social media. No amount of community benefit is worth your wellbeing.

Report abuse when it crosses platform guidelines. Document what you report (screenshot before reporting in case accounts get deleted). If harassment rises to the level of threats, stalking, or doxxing, consider involving law enforcement (though be realistic about how helpful they may or may not be).


The Comparison Trap#

Why Comparison Hurts You#

Comparison is perhaps the greatest psychological hazard of surgical recovery, and online communities intensify it dramatically.

Inequitable starting points make comparison meaningless. Your pre-operative anatomy isn't the same as someone else's. Your skin elasticity, scarring tendency, fat distribution, healing speed—none of these match anyone else's. Comparing outcomes when inputs differ is statistically illiterate.

When I see people post results and other users comment "wow, I wish mine looked like that," I want to shake them and say: that person had different tissue. A different surgeon. A different technique. Different genetics. You cannot will your outcome to match theirs any more than you can will yourself taller.

Different goals make comparison meaningless. Someone whose priority was standing urination and someone whose priority was aesthetic appearance will evaluate the same outcome completely differently. "Better" according to what criteria?

Genetic variation determines enormous amounts of healing trajectory that no one controls. Some people scar invisibly while others form keloids regardless of technique (Gauglitz et al., 2011). Some people's swelling resolves in weeks while others take months. Skin pigmentation affects scar visibility. Tissue elasticity affects results. Age affects healing speed and outcomes. Overall health affects everything.

You cannot will your genetics to be different.

Everyone's Timeline Is Different#

Recovery timelines published by surgical centers represent averages, and no individual is an average.

Swelling reduction varies enormously. Someone posting "look how good I look at six weeks!" may have genetic advantages in healing, may have photographed themselves at a particularly good moment in a fluctuating process, or may have a different procedure/technique with different typical timelines.

Scar maturation varies. Scars typically look worst at 3-6 months (red, raised, firm) then gradually fade and soften over the following year. But this timeline is highly individual. Comparing your three-month scars to someone else's one-year results isn't comparison—it's nonsense.

Sensation return varies. Nerves regenerate at their own pace, influenced by surgical technique, individual anatomy, age, and factors no one fully understands. Some people have significant sensation at three months; others take two years.

Yours is okay. Whatever your timeline, it's your timeline. As long as you're healing in the direction of recovery—confirmed by your surgical team—comparison with others' timelines tells you nothing useful.

Strategies for Avoiding the Trap#

Limit community engagement during vulnerable periods. The first few weeks of recovery, when you're most emotionally raw and your results look least like final outcomes, may not be the best time to scroll through other people's results photos. You can return to communities later when you're more stable.

Curate your feeds. Mute or unfollow accounts that trigger spiraling comparison. You can always re-follow later.

Focus on your own before-and-after. The only meaningful comparison is you versus your previous self. How do you look compared to week one? Week four? Your pre-operative state?

Remind yourself what you're seeing isn't reality. Those photos are moments—favorable moments, captured with intention. They don't show the swelling that came back an hour later, the bad days, the angles that looked worse.

Ask yourself what comparison gives you. If the answer is "anxiety and dissatisfaction," is that a good use of your time and emotional energy?

Talk to your therapist or support system about comparison when it happens. Sometimes saying it out loud—"I saw someone's results and now I feel terrible about mine"—helps break the spell.

Genetic Factors You Can't Control#

Some outcomes are genuinely beyond anyone's control. This is worth sitting with, because accepting it reduces suffering.

Keloid tendency is largely genetic. If your body forms raised, thickened scars, no amount of perfect surgical technique or diligent aftercare will change that fundamental tissue response (Gauglitz et al., 2011). You can manage it with silicone sheets, steroid injections, and other treatments, but you can't make your tissue respond like someone who doesn't have keloid tendency.

Scar visibility depends on skin tone, tissue characteristics, and healing patterns that vary by individual and can't be predicted with certainty beforehand. Darker skin tones often show scars more prominently through hyperpigmentation; lighter skin tones may show pink/red scars more vividly early in healing. Neither is better or worse—they're different.

Healing speed varies by age, overall health, nutrition, circulation, and genetics. Slower healing doesn't mean something went wrong—it means your body works at its own pace.

Tissue characteristics affect what's surgically possible and how results look. Your surgeon works with the material you provide; they cannot create tissue characteristics you don't have.

Understanding this can actually be freeing. If some things are beyond your control, you can stop blaming yourself for them. You can stop trying to will different outcomes through sheer determination. You can accept what is and work with it rather than against it.


Contributing to Community Knowledge#

The Value of Shared Experience#

If you choose to contribute your documentation to community resources, that contribution carries real weight. Shared surgical experiences create tangible benefits for future patients.

Setting realistic expectations through diverse outcomes happens when surgeon portfolios show only optimal results and community resources show the full range—including complications, suboptimal outcomes, and the messy middle of healing—so future patients can calibrate their expectations realistically.

Enabling informed surgeon selection through collective reviews helps future patients make better choices than they could from marketing materials alone through community consensus on surgeon quality, responsiveness, complication rates, and bedside manner.

Preparing for recovery through practical timeline information helps people prepare for their own journeys when they know what week three actually looks like, how long swelling actually takes to resolve, and what complications other people encountered.

Providing emotional support during stigmatized medical experiences matters because for many trans people, online communities are the only place they can discuss their surgeries openly. Contributing your experience helps others feel less alone.

TransBucket, founded in 2009, explicitly states its purpose: documenting outcomes "for everyone, from FTMs to MTFs to genderqueers, gender non-conforming individuals, agender individuals" including those who identify as detrans or desisted (TransBucket, n.d.).

I contribute to communities because I remember what it was like to have almost no information about nulloplasty. I remember searching desperately for anyone who'd had this procedure and could tell me what to expect. The scarcity of information made my journey harder than it needed to be. I don't want others to face that same scarcity if I can help fill the gap.

The Ethics of Sharing#

The ethical framework for contribution balances autonomy, beneficence, and privacy.

Autonomy means the AMA Code of Medical Ethics grounds medical sharing in informed consent and patient self-determination (American Medical Association, 2022). You have complete control over what, how, and with whom you share. No one is entitled to your story.

Beneficence asks whether sharing creates more good than harm, for both yourself and the community. Contribution that helps others while causing you distress isn't automatically worth it. Your wellbeing matters too.

Privacy means what you share cannot be unshared. Content persists, spreads, gets screenshotted and redistributed. The permanence is real.

Research on narrative medicine acknowledges that "sharing stories is a way to connect to others, to adopt a vulnerability that allows us to see and be seen more clearly. Because of this emotional vulnerability, stories can be dangerous, too" (Charon, 2017, p. 65).

When to Consider Contributing#

Consider waiting until you're emotionally settled and fully healed before contributing documentation. Early recovery is emotionally volatile, and decisions made during that period may not reflect your long-term preferences. Results aren't final until healing completes—sharing early photos without context may be misleading. You don't know yet what you'll want to be public with in five years.

Many people wait months or years before contributing. There's no rush.

Questions to ask yourself before contributing: Am I permanently comfortable with this being public? Do I understand that I cannot fully retract this even if I change my mind? Is my motivation to help others, or am I seeking validation/attention? (Both are valid, but know which you're doing.) Am I in an emotionally stable place to handle the responses I might receive? Have I protected my identity adequately?

How to Contribute Responsibly#

If you choose to share, do so thoughtfully.

Anonymize while maintaining useful information. Remove identifying features but preserve details that help others: surgeon name, technique, timeline, complications. Anonymous documentation that includes "had surgery in 2024" is less useful than documentation that specifies "had nulloplasty with Dr. X at the surgical practice in August 2024 using technique Y."

Provide context. A photo alone is incomplete. What was the timeframe? What technique was used? Were there complications? What would you do differently? Context makes your contribution useful rather than just another image.

Frame your experience as individual rather than universal. Your outcome is one data point, not a prediction of what others will experience. "This was my result" is accurate; "this is what happens with this surgery" is not.

Be honest about challenges alongside positive outcomes. Sanitized accounts of uncomplicated recoveries don't serve future patients navigating complications. If you had problems, saying so helps others feel less alone when they do too.

Update with long-term follow-up when possible. One-week results and one-year results tell different stories. Both matter. If you can share progression over time, that's more valuable than a single snapshot.

Engage respectfully with others' contributions. Remember that you're looking at real bodies belonging to real people. "It's easy to forget you're looking at real bodies, which belong to real human beings with real feelings. What you say has real consequences" (TransBucket, n.d.).


Making Your Own Choices#

You Don't Owe Anyone Your Story#

This deserves its own section, stated plainly: Your surgical journey is private. Sharing is optional. You get to set boundaries. You owe no one your documentation, your story, or your body.

You don't owe documentation to the community. "Future patients need resources!" Maybe, but you're not obligated to be that resource. Plenty of people share; if you don't, the community will survive.

You don't owe explanation to family members. "But we're worried about you!" They can be worried without access to intimate details of your medical care.

You don't owe proof to anyone who questions your choices. "How do I know you're really trans?" is not a question that deserves engagement.

You don't owe visibility to a movement. "Trans visibility helps everyone!" Maybe, but individual trans people are not obligated to sacrifice their privacy for collective benefit.

Your journey is private. It's medical information about your body. No one has a right to it without your consent.

Sharing is optional. If you never post a single photo, never write about your experience, never contribute to any community resource—you're not failing anyone. You're exercising your right to privacy.

You get to set boundaries. You can share some things and not others. You can share with some people and not others. You can change those boundaries whenever you want.

"I don't want to" is a complete reason. No justification required.

Selective Sharing Is Healthy#

Treating sharing as all-or-nothing misses the nuance of real life.

Choosing who to tell is your prerogative. Some people get access to everything. Some people get selected information. Some people get nothing. You calibrate based on relationship, trust, and what serves you.

Choosing what to share allows you to maintain privacy while still receiving support. You can share the fact of surgery without photos. You can share recovery challenges without showing your body. You can share outcomes without revealing complications. You can share with one person and ask them to relay updates to others so you don't have to repeat yourself.

Changing your mind is always allowed. If you shared something and now wish you hadn't, you can stop sharing more. (You may not be able to undo what's already out there, but you can stop adding to it.) If you were private and now want to share, you can start.

Different platforms have different audiences. What you share on a private Discord server might differ from what you'd share on public Reddit. What you tell close friends might differ from what you tell extended family. Calibrate to context.

Changing Your Mind Is Okay#

Your feelings about documentation and sharing will evolve. That's normal and healthy.

Started public, want to go private: Many people share freely in early recovery when seeking support, then pull back as they settle into post-surgical life. Reducing your visibility is always allowed. Delete what you can, make accounts private, and move forward.

Started private, want to go public: If you processed privately first and now want to contribute to community resources, that door is open. You can share at whatever point feels right.

Deleting posts is your right. You can't control screenshots or caches, but you can remove what's within your direct control.

Regretting what you shared doesn't mean you made a wrong decision at the time—it means your circumstances or feelings changed. Be compassionate with your past self.

Creating a Private Support System#

You don't need public community to have support during surgical recovery. Alternatives exist.

Close friends and family who understand can provide everything you need—emotional support, practical help, someone to process with. Quality matters more than quantity. One person who truly gets it is worth more than a hundred internet strangers.

Therapists and counselors provide professional support with legal confidentiality protections. If you don't have a therapist, surgical recovery is a reasonable time to find one. Many offer telehealth appointments accessible during recovery.

Private support groups exist on Discord, Signal, and other platforms with controlled access. These offer community connection without public exposure.

Individual connections made through communities can move to private communication. You might find a few people whose experiences resonate with yours and form private relationships with them—direct messages, text, phone calls—without maintaining public presence.

In-person support may exist in your area. Trans support groups, LGBTQ centers, and community organizations sometimes offer peer support for people navigating medical transitions.


Practical Checklists#

Before Surgery: Documentation Setup Checklist#

Photography infrastructure involves identifying your photo location with consistent lighting, marking floor position with tape if helpful, acquiring a tripod/phone mount if using one, getting a paper ruler for scale reference, testing your setup with practice photos, and documenting your setup (location, lighting, time of day, camera settings) for replication.

Digital security involves disabling automatic photo backup on all cloud services, installing a metadata removal tool (ExifCleaner, ExifTool, Metapho, or Scrambled Exif), setting up encrypted storage (Ente, Cryptee, Proton Drive, or VeraCrypt container), creating a folder structure for organizing documentation, and testing the full workflow: take photo, strip metadata, move to encrypted storage.

Journaling infrastructure involves choosing your format (paper journal, digital notes, voice memos), setting up a tracking log template (pain scale, medications, etc.), and creating a questions-for-doctor document.

Communication involves confirming the preferred method for sending photos to your surgical team, saving surgical team contact numbers, and knowing your patient portal login.

First Week Post-Surgery: Daily Documentation Protocol#

In the morning, take your temperature and record it in the log, note your pain level (0-10 scale), record any medications taken, and note wound observations.

During your clinical photo session (once daily, at consistent time), set up in your established photo location, take a medium shot with scale reference, take close-up shots, review photos immediately for quality, strip metadata, and move to encrypted storage with proper filename.

Throughout the day, track medication doses and times, note drain output amounts and colors (if applicable), record bowel movements, note food/fluid intake, track any concerning symptoms, and write down questions for your surgical team as they arise.

In the evening, take your temperature and record it in the log, note your pain level, and write a brief journal entry (optional but recommended).

Signs Requiring Immediate Documentation and Surgeon Contact#

Photograph and contact your surgical team immediately if you notice temperature above 101°F (38.3°C) especially with chills, spreading redness extending beyond the surgical site, increased or discolored discharge (yellow, green, brown), foul odor from the surgical site, wound opening or separation, tissue color changes toward black or gray, sudden increase in pain not controlled by medication, bright red bleeding soaking through dressings, signs of blood clot (leg swelling, warmth, pain), difficulty breathing or chest pain, or inability to urinate (if catheter-free).

When contacting your surgical team, have ready your name, surgeon's name, and date of surgery; specific symptom description including when it started; temperature reading; current medications; and photos of concerning areas.

If you take nothing else from this guide, take this: set up your systems before surgery. The acute post-operative period is not the time to be learning about metadata removal or figuring out how to create encrypted storage. Do that work now, while you have the energy and clarity. Future you will be grateful.

And remember: whatever you choose about documentation and sharing, that choice is valid. Complete privacy is valid. Selective sharing is valid. Open contribution is valid. The only wrong answer is doing something you'll regret because you felt obligated. This is your body, your journey, your story. You decide who knows it.
