Pain is an expected part of recovery. How you manage it significantly affects your experience and your ability to rest, eat, move appropriately, and do the other work of healing.
Medication Schedules#
Stay ahead of pain rather than chasing it. When you wait until pain becomes severe before taking medication, you're allowing pain to escalate to levels that are harder to bring back down. Taking scheduled medications on time, even if pain seems manageable at the moment, prevents peaks of severe pain that break through medication levels.
Set alarms for medication times. When you're tired, medicated, and losing track of time, missing doses is easy. Alarms—on your phone, or from a caregiver—prompt timely doses.
Track what you've taken. A simple written log of medication name, dose, and time prevents accidental double-doses or missed doses. Some people use apps designed for medication tracking; others use a notebook.
A multimodal approach combines different types of pain medications that work through different mechanisms. Common regimens include acetaminophen (Tylenol, up to 3000 mg daily unless otherwise instructed—watch for acetaminophen in combination medications to avoid exceeding limits) plus NSAIDs like ibuprofen if not contraindicated (some surgeons prohibit NSAIDs due to bleeding risk—follow your team's guidance) plus opioids at the lowest effective dose for breakthrough pain.
Using multiple medications allows lower doses of each, often reducing side effects while providing better overall pain control. Your surgical team will prescribe an appropriate regimen for your procedure.
Never combine opioids with alcohol, benzodiazepines, or other sedatives. This combination can cause respiratory depression—slowed breathing that can be fatal. If you take any regular medications with sedating effects, discuss this with your surgical team before surgery.
This is a lot to take in. Pain management sounds complicated, but your surgical team will give you specific instructions. Your job is to follow them, set alarms for medication times, and ask for help if pain isn't controlled. You don't need to become a pharmacology expert—you just need to take what's prescribed, when it's prescribed.
Non-Pharmaceutical Approaches#
Ice and cold therapy reduce swelling and numb pain during early recovery when inflammation is highest. Gel packs wrapped in cloth (never direct ice on skin), applied for 20 minutes on and 20 minutes off, provide relief without tissue damage. Ice is most helpful in the first week or two after surgery.
Heat therapy becomes more appropriate in later recovery, when acute inflammation has resolved and chronic discomfort or muscle tension is the issue. Heating pads, warm baths (once cleared for soaking), or microwaveable heating packs can soothe soreness. Heat promotes blood flow and muscle relaxation.
Positioning affects pain significantly. Experimenting with pillow placement, body angle, and how you hold yourself can reduce strain on surgical sites. Small adjustments sometimes make surprising differences.
Some people use breathing, relaxation, or meditation to feel less overwhelmed by pain or anxiety. The cited review found low-quality evidence of a small pain decrease for mindfulness meditation in chronic-pain trials; it does not establish immediate postoperative benefit from every named exercise (Hilton et al., 2017). Use any practice as optional support, not as a substitute for the team’s pain or complication guidance.
Distraction genuinely reduces pain perception. Engaging your brain with television, books, games, podcasts, or conversation occupies neural resources that would otherwise focus on pain signals. This isn't "ignoring" pain—it's a legitimate pain management strategy used in clinical settings.
Managing Breakthrough Pain#
Breakthrough pain—spikes of pain that occur despite regular medication—is normal and doesn't mean something is wrong. Some activities, times of day, or positions predictably increase pain.
If breakthrough pain occurs regularly, discuss with your surgical team. They may adjust your medication schedule, add a short-acting medication for specific activities (like before dilation or physical therapy), or recommend additional strategies.
Know what's normal versus concerning. Pain that is increasing after the first three to five days warrants attention—healing generally follows a pattern of gradually decreasing pain, and reversal of this pattern can signal complications like infection or hematoma. Sharp, severe pain that's new and different from your baseline pain should prompt immediate contact with your surgical team.
Weaning Off Pain Medications#
The goal is to reduce opioids as soon as reasonably possible while maintaining adequate pain control. Opioids carry risks including dependence, constipation, cognitive impairment, and other side effects that make tapering appropriate as healing progresses.
The timeline for weaning varies enormously by procedure and individual. Some patients transition off opioids within one to two weeks; others require them longer. Many patients report less pain than anticipated after certain procedures, managing with acetaminophen or NSAIDs alone beyond the first week. Don't compare yourself to others—your pain is your own.
Gradual reduction prevents withdrawal symptoms and allows you to assess pain management at each level. A typical approach might involve taking one less dose per day for several days, then one less again. Your surgical team can provide a suggested taper schedule.
Transition to over-the-counter options as opioids decrease. Acetaminophen and NSAIDs (if permitted) can manage moderate pain effectively without opioid side effects.
Withdrawal symptoms from extended opioid use can include flu-like symptoms, anxiety, insomnia, and discomfort. These are uncomfortable but not dangerous. If you experience concerning symptoms while tapering, contact your surgical team.
Managing Constipation#
Constipation is one of the most preventable sources of recovery misery, yet it catches countless patients off guard.
Start stool softeners with your first opioid dose. Don't wait for constipation to develop. Docusate sodium (Colace) taken regularly softens stool and makes passage easier.
If stool softeners alone aren't sufficient after 24-48 hours without a bowel movement, add a stimulant laxative like senna (Senokot). Stimulant laxatives promote intestinal contractions that move things along.
Hydration and fiber intake work with medications to prevent constipation. Adequate fluids keep stool soft; adequate fiber provides bulk that promotes regular motion.
Contact your surgical team if you haven't had a bowel movement in three or more days. Severe constipation can cause significant discomfort and, in extreme cases, complications. Early intervention is preferable to waiting until the situation becomes urgent.
Straining during bowel movements can damage fresh surgical sites. Preventing constipation prevents straining. This is not a minor concern—it's a direct surgical outcome matter.
Movement and Physical Activity Timeline#
Movement during recovery follows a carefully staged progression. Too little activity risks blood clots and stiffness; too much risks damaging healing tissue. This section provides general guidance, but always follow your specific surgical team's instructions.
Immediate Post-Op: Minimal Movement#
During the first days after surgery—particularly while hospitalized—your movement will be limited but not zero. What movement occurs matters for preventing serious complications.
Gentle repositioning in bed, shifting your weight and position, prevents pressure sores and promotes circulation. Nursing staff will help you reposition regularly.
Major surgery can require a formal VTE-prevention plan, including pharmacologic and/or mechanical measures selected by the care team (Geerts et al., 2008). Do not treat unsupervised foot or calf movements as adequate prophylaxis. Ask the team which movement, devices, medicines, mobility restrictions, and urgent symptoms apply to your operation.
Coughing and deep breathing exercises prevent pneumonia and other respiratory complications that can occur when patients breathe shallowly due to pain. A pillow held against your abdomen or chest can provide support and reduce discomfort during coughing.
For some procedures, strict bed rest may be required for the first several days. Vaginoplasty and phalloplasty often involve minimal activity in the first week; follow your specific instructions carefully.
Week Two: Gentle Walking#
Once cleared to walk—typically after the first week for many procedures—begin with short, slow walks. Walking promotes circulation, prevents blood clots, supports digestion and bowel function, and provides mental health benefits.
Start with distances measured in feet, not blocks. Walking from your bed to the bathroom and back may be the appropriate initial goal. Gradually increase distance as tolerated, adding perhaps the length of a hallway or a room.
Walk with support if you feel unsteady. A caregiver nearby, a walker, or furniture to steady yourself on prevents falls during this vulnerable period.
Speed doesn't matter. You're not exercising; you're moving blood through your body and gently reminding your legs how to work. Slow is fine.
Weeks Three Through Six: Increasing Activity#
During weeks three through six, most patients experience gradual improvement in mobility and stamina. Walking distances increase; some people feel ready for short outings.
Stairs may be introduced during this period, with caution. Start with a few stairs at a time, using handrails, going slowly. Don't rush stair progression.
Light activities become possible: cooking simple meals, brief social visits, short car rides as a passenger. Energy remains limited; plan for significant rest periods between activities.
Avoid anything that feels like straining, lifting, or pushing your limits. The body is still healing internally even as external wounds look better.
Returning to Exercise#
Return to exercise follows a staged progression that varies significantly by procedure. In general:
Light walking continues to increase throughout months two and three.
Low-impact activity—swimming, stationary cycling, gentle yoga—may be cleared around two to three months, depending on procedure and individual healing.
Strength training begins at three to six months for most procedures, starting with light weights and avoiding strain on surgical areas.
High-impact activity—running, jumping, contact sports—typically waits until three to six months or longer, depending on procedure.
Always get explicit clearance from your surgical team before advancing to new activity levels. What feels ready may not be healed enough; their assessment matters more than your enthusiasm.
Procedure-Specific Restrictions#
Top surgery: No heavy lifting, typically defined as more than five to ten pounds, for about four to six weeks. Arm movement restrictions vary by technique but often include avoiding raising arms above shoulder height for several weeks. Sleep elevated on your back for approximately six weeks. Compression garments are worn for several weeks as directed. Drains are typically removed at the first post-operative appointment (around day six or seven) once output drops below 20-30ml per 24 hours—this milestone allows showering and reduces daily caregiving intensity.
Vaginoplasty: No sitting on flat surfaces during weeks one through four—lie down or stand only, using a donut cushion when sitting becomes permitted. No penetration of any kind until explicitly cleared by your surgeon. Strict activity limitations for six to eight weeks. Dilation requirements take priority over other activities. Catheters are typically removed around day eight.
Phalloplasty: Largely bed-bound during early recovery. Phallus must be elevated and positioned as directed, often at specific angles. Avoid any pressure on groin vessels. Multiple-stage procedures involve repeated recovery periods with stage-specific restrictions. The VAC dressing is usually removed around day five, and wound care transitions to daily dressing changes.
Nulloplasty: Strict activity restrictions in the first week—no sitting on flat surfaces (use donut cushion when sitting becomes permitted), limited walking, and careful positioning to avoid pressure on the surgical site. Catheter removal usually occurs within the first two weeks. This milestone marks a significant shift—regaining control over urination after days of catheter management feels enormous. Some patients experience temporary spraying or difficulty controlling stream; this typically resolves as swelling decreases and you learn your new anatomy.
Hysterectomy: No heavy lifting for six weeks or as directed. Gradual return to activity, with particular attention to any internal sensations that suggest trouble.
Facial surgery: Sleep elevated for swelling reduction. Protect the face from any contact or pressure. Sun protection for scars is particularly important on exposed facial areas.
Sleep and Rest#
Sleep is when your body does its most intensive repair work. Growth hormone release during deep sleep promotes tissue healing; immune function operates optimally during rest. Prioritizing sleep isn't indulgence—it's direct investment in your surgical outcomes.
Sleeping Positions by Procedure#
Your procedure dictates how you must sleep, at least initially:
Top surgery: Elevated sleeping on your back, typically using a wedge pillow or recliner. Side sleeping is generally prohibited until cleared by your surgeon.
Vaginoplasty: Back sleeping, often with legs supported by pillows. Side sleeping may be permitted after initial healing; ask your surgeon about timing.
Phalloplasty: Back sleeping with the phallus elevated and positioned as directed—often requiring specific pillow arrangements.
Nulloplasty: Back sleeping with careful positioning to avoid pressure on the surgical site. Pillow arrangements that keep you from rolling onto your stomach or putting pressure on healing areas are essential.
Hysterectomy: Elevated sleeping on the back is common initially, with gradual return to preferred positions.
Facial surgery: Elevated sleeping to reduce swelling. Keep your head above your heart.
I've talked with many community members who found mandatory back sleeping to be one of the most challenging aspects of recovery—particularly those who were lifelong side or stomach sleepers. Using a body pillow to create a wedge that prevents rolling, or sleeping in a recliner, can help maintain required positions.
Managing Sleep Disruption#
Sleep during recovery is often poor despite how much your body needs it. Pain wakes you; medication schedules disrupt continuous sleep; catheter discomfort prevents relaxation; anxiety and hypervigilance keep your nervous system activated. This is normal, though frustrating.
Pain interrupting sleep may indicate that medication timing needs adjustment. If you're consistently waking due to pain in the early morning hours, discuss with your team whether a different dosing schedule might provide better overnight coverage.
Medication side effects—particularly from opioids—can paradoxically disrupt sleep through restlessness, vivid dreams, or uncomfortable sweating.
Catheter discomfort prevents restful sleep for many patients. Adjusting the tubing, ensuring the bag is positioned below bladder level without tugging, and using additional pillow support around the catheter site can improve comfort.
Strategies for better recovery sleep include: keeping the room cool (60-67°F is optimal for most people); eliminating electronics and screen use for an hour before sleep; maintaining a consistent sleep schedule even though you may nap during the day; separating from your caregiver if their presence is disrupting—some patients find that having a caregiver in a separate room with a baby monitor provides better rest for both parties while maintaining safety.
Accept that sleep will likely be poor during initial recovery. This is temporary. Don't add anxiety about poor sleep to your challenges; do what you can and trust that rest will improve as healing progresses.
The Importance of Rest#
Even when awake, rest matters. Lying down, relaxing, minimizing demands on your body—all of this supports healing.
Resist the urge to be productive. Recovery is not the time to catch up on projects, prove yourself capable, or set impressive records for how quickly you bounced back. Those instincts serve you in other contexts; right now, they work against healing.
Give yourself permission to rest. This is your job right now. You are not being lazy; you are doing exactly what your body needs. When guilt about resting arises—as it does for many of us—remind yourself that healing is an active process requiring energy and resources. Rest is how you provide those resources.
Recovery IS your job now. Not your only life activity—you can also watch movies, talk to friends, read, and enjoy small pleasures. But recovery is your work, and rest is the primary way you accomplish that work.
Managing Post-Op Depression and Blues#
Emotional distress during recovery is common, often unexpected, and doesn't mean something is wrong with you or your surgery. Understanding why it happens and how to cope helps you navigate this aspect of healing.
Why It Happens#
Post-operative blues and depression arise from multiple converging factors:
Hormonal changes occur immediately when hormone-producing organs are removed or altered. Orchiectomy (removal of testes) during vaginoplasty dramatically changes hormonal balance. Even when you're on hormone replacement therapy, the transition period can affect mood. For those with ovaries, hysterectomy causes hormonal shifts regardless of whether hormone therapy is used.
Physical pain, fatigue, and discomfort can be hard to carry during recovery. The cited review describes an association between chronic pain and depression; it does not predict a short-term postoperative mood course (Sheng et al., 2017). Tell the surgical, prescribing, primary-care, or mental-health team about new, persistent, or frightening mood changes.
General anesthesia effects on brain chemistry can persist for days to weeks. Postoperative cognitive dysfunction—difficulty concentrating, memory issues, emotional lability—affects somewhere between 11% and 63% of patients depending on the study and type of surgery. Many patients report cognitive fog, emotional flatness, or mood disturbance in the weeks following general anesthesia.
Opioid and other medication side effects can affect sleep, concentration, energy, and mood for some people, but the retrieved evidence does not establish that 10% of people taking opioids for more than a month develop depressive symptoms. New, persistent, or frightening mood changes deserve a call to the prescribing or surgical team rather than self-diagnosis from a percentage.
Life disruption—the inability to work, care for yourself, or engage in normal activities—challenges identity and autonomy. Many of us derive self-worth from productivity; being forced to be taken care of can trigger uncomfortable feelings. You're not doing the things that normally help you feel like yourself—going to work, seeing friends, pursuing hobbies, moving your body. You're dependent on others in ways that may feel uncomfortable. You're spending long hours with nothing to do but think, and your thoughts may not be kind company.
Identity processing accompanies major body changes, even desired ones. Your body is different now. Even when you wanted this, integrating the change into your sense of self takes time. Parts of you may grieve what's gone, even as other parts celebrate what you've gained.
Body changes themselves, even when desired, can temporarily destabilize self-perception. Looking at unfamiliar anatomy, healing wounds, and swelling that obscures results can all trigger distress. Your self-image is updating. Your relationship to your body is shifting. That process can be disorienting even when it's ultimately positive.
Emotional Timeline#
Research and clinical experience suggest a typical pattern, though individual experiences vary:
Days 1-7 often involve exhaustion, confusion from medications, anxiety about outcomes, sadness that may feel irrational, irritability, and sometimes doubt about whether you made the right choice. These feelings don't mean you made the wrong decision—they mean you're human, recovering from major surgery. Many community members describe this first week as the hardest emotionally, even harder than the physical challenges. Crying spells that don't necessarily correspond to negative feelings are common—sometimes you cry because you're relieved, because it's over, because you can't believe you actually did this.
When I woke up from my own surgery, my first moment of awareness brought relief so profound it made me cry before I was fully conscious. But that initial relief gave way to days of emotional volatility—tears arriving unexpectedly, sometimes from relief, sometimes from the sheer overwhelm of having actually done this thing I had wanted for so long. The euphoria came, but it came in waves interspersed with exhaustion, frustration, and moments of "is this supposed to look like this?" That's normal. The emotional rollercoaster is part of the process.
Days 8-14 frequently bring hyper-critical self-assessment. Patients scrutinize surgical results anxiously, often catastrophizing about swelling or early healing appearances that are entirely normal. Impatience with the healing timeline peaks; everything seems to be taking too long; despair about ever feeling normal again can arise. This is often when the initial crisis energy fades and the long slog of recovery becomes apparent.
Days 15-21 often mark a turning point. Many patients begin to notice results emerging as initial swelling decreases, feel more like themselves, experience more moments of genuine positivity. Energy begins returning. Mood stabilizes.
Weeks to months see continued gradual emergence of gender euphoria and satisfaction for many patients. As swelling resolves and healing progresses, the reality of what surgery accomplished becomes more apparent. Full emotional processing of the surgical experience continues. Integration of body changes into self-image is ongoing. Most people report that their emotional state is substantially better than before surgery by two to three months post-op, with continued improvement over the first year. For many, this is when the profound meaning of the change really lands.
Long-term research can describe associations without predicting an immediate recovery course. In a secondary analysis of the 2015 U.S. Transgender Survey, 3,559 respondents who reported one or more gender-affirming surgeries at least two years earlier had lower adjusted odds of past-month severe psychological distress (aOR 0.58) and past-year suicidal ideation (aOR 0.56) than 16,401 respondents who desired surgery but had not had it (Almazan & Keuroghlian, 2021). Those adjusted observational associations are not proof of 42% or 44% causal reductions, and they do not establish when mood, relief, dysphoria, or euphoria will change after an operation.
How to Recognize When It's More Than Blues#
Emotional struggle during recovery can deserve professional attention at any point; no general week-count separates ordinary distress from a need for help. Contact the surgical, prescribing, primary-care, or mental-health team when low mood, anxiety, sleep or appetite changes, substance use, or difficulty with required self-care is worrying you or interfering with recovery. Seek emergency help immediately if you may act on thoughts of self-harm, cannot stay safe, or have an acute medical emergency.
Warning signs include: persistent low mood lasting more than a few weeks without improvement; loss of interest in activities you usually enjoy, including things unrelated to surgery that you can do during recovery like watching shows or talking to friends; difficulty sleeping beyond what pain causes (sleeping constantly when you're not particularly tired, insomnia from racing thoughts, or early waking); significant appetite changes beyond normal recovery fluctuations; feelings of hopelessness, worthlessness, or excessive guilt; inability to perform required self-care tasks (wound care, dilation) due to lack of motivation; increasing isolation or withdrawal from support system; and especially any thoughts of self-harm or suicide.
Suicidal ideation requires immediate response. If you're having thoughts of wanting to die, thinking the world would be better without you, or considering methods of self-harm, this is a crisis requiring immediate support. Contact a crisis line, tell your caregiver, go to an emergency room, or call 911. Do not wait. Do not minimize. Do not try to manage it alone.
I need to be honest about something here: sixteen months after my initial nulloplasty, when I was hospitalized with sepsis following a revision surgery, alone in San Francisco, thousands of miles from my husband KJ, I experienced suicidal ideation. The despair was real. The thought that I didn't want to be alive anymore was real. The compounding factors—physical pain, emotional isolation, financial stress, being far from everyone I loved, still in early recovery from alcohol, high-potency IV pain medications—created a perfect storm that carpet-bombed whatever mental health I had left.
I'm telling you this because if you find yourself in a dark place during recovery—whether from your initial surgery or from complications that arise later—you should know: it happens to people who wanted their surgery deeply and would do it again in a heartbeat. Darkness during recovery doesn't mean regret. It doesn't mean something is wrong with your character. It means you're in crisis and you need support. Get help. Call someone. You don't have to manage it alone, and you shouldn't try.

These resources exist because people in crisis deserve support. Using them is appropriate and often life-saving.
Coping Strategies#
Talk about what you're feeling. Keeping difficult emotions bottled up amplifies them. Tell your caregiver, call a friend, post in a support group, talk to a therapist. The act of putting feelings into words helps process them. You don't have to present a brave face to everyone—find at least one person you can be honest with.
Stay connected even when you don't feel like it. Social connection is the strongest protective factor against depression. Video calls with friends, text conversations, online community participation, visits from people you're comfortable seeing in your current state—all of it helps. Isolation feeds depression; connection interrupts it.
Get outside when you can. Natural light supports circadian rhythm and mood. Even sitting by a window helps. When you're mobile enough, short walks outside provide the combined benefits of light, movement, and change of environment.
Do small things you enjoy. You may not have energy for much, but small pleasures add up: a favorite show, comfort food you actually want to eat, music you love, a phone game that passes the time. Don't judge yourself for what counts as enjoyment right now—if it provides any positive feeling, it's serving a purpose.
Maintain basic routines. Getting dressed instead of staying in pajamas all day, washing your face, eating at regular times—these small structures provide psychological scaffolding when everything else feels chaotic.
Practice self-compassion deliberately. Notice self-critical thoughts and consciously counter them. You're recovering from major surgery. You're allowed to feel bad. You're allowed to need help. You're allowed to not be okay for a while.
Be gentle with yourself throughout this process. Treat yourself with the compassion you'd offer a friend in the same situation. Self-criticism and high expectations worsen depression; self-compassion supports recovery.
Creative expression—journaling, art, writing, music—provides outlets for processing complex feelings and documenting the journey. Many community members find that keeping a recovery journal helps them see progress over time and process difficult emotions.
When to Seek Help#
If emotional distress is severe, persistent, or involves any thoughts of self-harm, professional help is warranted. This is not weakness; this is wisdom about when to bring in additional resources.
Contact your primary care provider or surgical team about mood disturbances. They may recommend mental health referrals, medication adjustments, or evaluation for post-surgical depression.
Therapy or counseling during recovery provides space to process the emotional complexity of major surgery. Many people benefit from talking to a mental health provider who understands gender-affirming care.
Medication for depression or anxiety, if appropriate, should not be avoided simply because you're "supposed to" feel good after desired surgery. If medication would help, it's worth discussing.
Scar Care#
Surgical scars are part of your healing story. Understanding how to care for them supports optimal healing and helps you develop realistic expectations about long-term appearance.
Timeline for Scar Treatment#
Early care (weeks 0-6): Focus on protecting wounds and allowing them to heal. Keep incisions clean, follow wound care instructions, protect healing tissue from sun exposure, and don't pick at scabs or sutures. This is not the time for scar treatment—wounds must be fully closed first.
Intermediate care (after full healing, typically 6+ weeks): Once wounds are completely healed, all scabs have fallen off naturally, and any sutures are removed or dissolved, scar treatment can begin. Discuss timing with your surgical team.
Long-term care (months to years): Scar maturation continues for one to two years. Consistent care during this period produces the best results.
Silicone Sheets and Gels#
Silicone-based products are one topical option people may discuss after complete healing. The cited review found limited, mostly low-quality clinical evidence for topical scar treatments, rather than a universal best treatment or guaranteed appearance change (Sidgwick et al., 2015).
Silicone sheets are sticky-backed strips applied directly to the scar. They're worn for extended periods—often 12-24 hours daily—and can be reused for days to weeks before replacement.
Silicone gels dry to form a protective layer and may be more convenient for areas where sheets don't adhere well or for use under clothing.
Start silicone treatment after full wound healing and continue for at least three months for best results. Some patients continue for six to twelve months.
Cost varies; products are available at pharmacies, online, and through medical supply stores. Generic options work as well as expensive brands.
Massage Techniques#
Scar massage softens scar tissue, improves flexibility, and may improve appearance by breaking up collagen bundles that form during healing.
Begin massage only after wounds are fully healed and with surgeon approval—typically at least three weeks post-op, often later.
Technique involves applying firm but gentle pressure to the scar in circular motions, then cross-friction (perpendicular to the scar direction), then lengthwise along the scar. Start with a few minutes daily and gradually increase.
Some tenderness is normal; sharp pain is not. If massage causes significant discomfort, wait longer before resuming.
Sun Protection#
UV exposure darkens scars and can make them more prominent. Scars are particularly susceptible to hyperpigmentation in the first year.
Keep scars covered or protected with SPF 30+ sunscreen whenever sun exposure is possible. This applies year-round—UV penetrates clouds and affects skin in winter.
Broad-spectrum sunscreen protects against both UVA and UVB rays. Reapply every two hours during continuous exposure.
Clothing provides the most reliable protection. If scars can be covered by fabric, this is preferable to sunscreen alone.
Maintain sun protection for at least twelve months, longer if scars remain red or raised.
Realistic Expectations#
Scars fade significantly over time—twelve to twenty-four months of maturation typically produces major improvement in color, thickness, and prominence.
Scars may never disappear completely. Most surgical scars remain visible upon close examination, even when optimally healed. This is normal and expected.
Skin tone affects scar visibility. People with darker skin tones may develop keloid or hypertrophic scars more commonly and may find that scars remain more visible long-term. People with very fair skin may have scars that remain red or pink longer.
Scar care improves outcomes but has limits. The most meticulous care can't guarantee invisible scars; genetics, skin type, and surgical technique all play roles beyond your control.
Your scars tell a story. For many trans individuals, surgical scars represent liberation, self-determination, and the culmination of a long journey. Whatever relationship you develop with your scars—pride, acceptance, indifference, or desire to minimize them—is valid.
When Can I...?#
Recovery involves slowly reclaiming normal activities. Timelines vary by procedure, individual healing, and surgical team guidance, but the following provides general orientation.
Common Questions by Activity#
Drive: When you're off narcotic pain medications (which impair reaction time and judgment), have adequate strength and range of motion to control the vehicle safely, and feel confident in your ability to respond quickly in an emergency. This is typically two to four weeks for minor procedures, six weeks or longer for major procedures.
Return to work: Depends enormously on job demands. Desk work from home may be possible in as little as two to four weeks for some procedures. Jobs requiring standing, walking, physical activity, or customer interaction typically require six to eight weeks or longer. Physically demanding jobs may require three months or more. Prior to returning, ensure you can maintain necessary self-care (like dilation for vaginoplasty) around work schedules.
Exercise: Light walking begins early and increases gradually. More intensive exercise resumes at three to six months depending on procedure. Always get explicit clearance before resuming significant exercise.
Have sex: Varies dramatically by procedure. For vaginoplasty, penetrative vaginal intercourse is typically cleared at eight to twelve weeks after surgeon evaluation confirms adequate healing. For other procedures, follow your surgeon's specific guidance. A minimum of six to eight weeks is common for most genital surgeries.
Swim or bathe in tubs: Wounds must be fully closed before submerging in water, which can introduce bacteria. Typically six weeks or longer. Hot tubs may require additional waiting time due to bacterial load in the water.
Lift heavy objects: Typically restricted for six weeks or longer for most procedures. "Heavy" often means more than 5-10 pounds during initial restrictions, gradually increasing as cleared.
Travel by plane: Air travel increases DVT risk and removes easy access to your surgical team if problems arise. Most surgeons advise against flying for four to six weeks post-surgery; longer for complex procedures. Always discuss travel plans with your surgeon before booking.
Procedure-Specific Timelines#
Vaginoplasty: Dilation schedule is lifelong, with frequency decreasing over time. Penetrative sex typically cleared around eight to twelve weeks. Sitting restrictions apply for the first several weeks; follow specific guidance on when flat sitting is permitted. Most patients return to light work at six to eight weeks; physically demanding work requires longer.
Phalloplasty: Multiple stages involve separate recovery periods. Even when home, activity restrictions are significant between stages. Full return to normal life may take twelve months or longer across all stages. Erectile device placement (for those choosing this option) comes later and has its own recovery period.
Top surgery: Arm restrictions and compression garment wear continue for several weeks. Light activity often possible at two weeks; exercise at six to eight weeks. Most patients return to work within two to three weeks for desk jobs.
Hysterectomy: Lifting restrictions for six weeks. Return to work at two to six weeks depending on job demands. Return to exercise at six to eight weeks.
Facial surgery: Swelling takes months to fully resolve; final results at twelve to twenty-four months. Return to work depends on comfort with appearance during visible healing. Sun protection and gentle skin care are ongoing.
Accessibility Considerations for Neurodivergent and Disabled Patients#
Recovery can be particularly challenging for patients who are neurodivergent or have disabilities. Planning and accommodations can smooth the process.
Sensory Considerations#
Hospital environments are often sensorily overwhelming: fluorescent lighting, constant noise, unfamiliar smells, uncomfortable bedding, and interrupted sleep. If you have sensory sensitivities, consider bringing items that help regulate your sensory experience: noise-canceling headphones, a sleep mask, earplugs, a familiar blanket or pillow, comfort textures.
Request a quiet room if possible. Hospitals may accommodate requests to be away from noisy areas like nurse stations or elevators.
Your recovery space at home can be modified for sensory needs: dimmable lighting or lamps instead of overhead lights; blackout curtains for light sensitivity; soft textured bedding; reduction of ambient noise.
Communication Preferences#
Clarify how you best receive information—verbally, in writing, or visually. Request written copies of all post-operative instructions; verbal explanations during the foggy post-anesthesia period may not stick.
If you have communication difficulties due to autism, auditory processing challenges, or other factors, inform your surgical team in advance. Many will accommodate by providing information in your preferred format.
Communication aids—text-to-speech, communication boards, or AAC devices—should be accessible during recovery if you use them normally.
Medication Management Strategies#
Managing multiple medications on complex schedules is demanding for anyone; it's particularly challenging with executive function difficulties.
Pill organizers with separate compartments for each dose time provide visual clarity and reduce decision-making load.
Alarms and reminders on phones or dedicated medication reminder devices prompt doses when time perception is unreliable.
Written schedules—a simple chart showing what to take when—remove reliance on memory.
Apps designed for medication tracking may help; examples include Medisafe, MyTherapy, and Round.
If executive function is likely to be significantly impaired during recovery, arrange for a caregiver to manage medications.
Managing Pain Medication Side Effects#
Opioids and other pain medications affect cognition, mood, and executive function. For those already managing neurodivergent brains or invisible disabilities, these effects compound.
Anticipate reduced executive function when planning for recovery. Decisions that seem simple—what to eat, when to take medications, whether to call the surgeon—become harder. Reduce the number of decisions required by preparing in advance.
Support for daily task completion may be necessary even for people who normally manage independently. Accept help during this period.
Accessing Emotional Support#
Identify mental health support needs before surgery. Will you need additional therapy sessions during recovery? Is telehealth available so you can access therapy from bed?
If you use a therapist or psychiatrist, schedule recovery check-ins before surgery while you have full cognitive function.
Online support communities can provide connection when leaving home isn't possible. Trans-specific forums, Discord servers, and support groups exist for peer support during recovery.
Working with Your Medical Team#
Clear communication with your surgical and medical team supports optimal outcomes. You are part of this team, and your participation matters.
Following Post-Op Instructions#
Your surgeon provides specific post-operative instructions based on your procedure, their technique, and their experience with what produces best outcomes. These instructions are not suggestions—they're the protocol that gives you the best chance of healing well.
Document instructions carefully. After surgery, with anesthesia and pain medications affecting your cognition, you won't remember verbal instructions reliably. Ensure you have written copies, and ask for clarification before leaving the hospital.
Follow instructions precisely, even when specific requirements don't seem obviously necessary. If you don't understand why something is required, ask—understanding improves adherence.
If you're struggling to follow instructions—for example, if the dilation schedule is impossible to maintain, or you can't tolerate a required medication—contact your team rather than silently deviating from protocol. They may have alternatives or modifications.
When to Contact Your Surgeon#
Signs of infection warrant contact: fever over 101°F (38.3°C), increasing redness or warmth at the surgical site, foul-smelling discharge, increasing pain rather than improving pain, and red streaks spreading from wounds.
Unusual pain patterns require attention: pain that was improving but suddenly worsens, pain significantly different from what you were told to expect, or pain not controlled by prescribed medications despite following instructions.
Concerns about healing are appropriate to raise: wounds that aren't closing, granulation tissue, unusual symptoms, or anything that seems different from what you were told to expect.
Questions about activity are valid: when in doubt about whether an activity is permitted, ask rather than guessing.
When you call, be prepared to describe symptoms specifically: when they started, how they've changed, what makes them better or worse, and what you've tried. The more specific information you can provide, the more useful guidance you'll receive.
When to Seek Emergency Care#
Some situations require emergency room care rather than waiting to reach your surgeon:
High fever with chills (104°F/40°C or higher, or fever with shaking chills) may indicate serious infection requiring immediate IV antibiotics.
Signs of blood clot in the leg (deep vein thrombosis): leg pain, swelling (often in one leg more than the other), warmth, or redness. If a clot travels to the lungs (pulmonary embolism), symptoms include sudden shortness of breath, chest pain especially when breathing, rapid heart rate, dizziness, or coughing blood. DVT and PE are medical emergencies.
Severe bleeding that doesn't stop with pressure or that results in soaking through dressings rapidly requires immediate evaluation.
Signs of flap compromise (for phalloplasty and some other reconstructive surgeries): changes in color (pale, dusky, or mottled), temperature (cold), or appearance of the flap. These may indicate vascular problems requiring emergency intervention.
Suprapubic catheter falls out: Go to the emergency room for replacement. The tract can close quickly, losing access.
If in doubt, it's better to seek emergency evaluation and learn everything is fine than to wait and allow a serious problem to worsen.
When I developed sepsis following my revision surgery, the signs came on gradually enough that I kept second-guessing whether I was sick enough to seek help. I noticed pain worsening instead of improving, overwhelming fatigue, and eventually a fever of 103 degrees. By the time I finally went to the emergency room, I was dangerously ill. Trust your instincts. If something feels wrong, it probably is.
Follow-Up Appointments#
Schedule follow-up appointments before surgery when possible, particularly the first post-operative visit. This ensures you have a date secured and reduces the cognitive burden of scheduling while recovering.
Bring a list of questions to appointments. It's easy to forget things when you're in the exam room; writing questions in advance ensures you cover what matters.
Transportation requires planning. You may not be able to drive yourself to early appointments. Arrange for a companion to provide transportation and potentially sit in on appointments to help you remember what was said.
Take notes or ask if you can record the conversation. Post-surgical cognitive fog can make it hard to retain information.
Celebrating Small Victories#
Recovery is long, and maintaining morale matters. Recognizing progress, even small progress, sustains motivation and mental health.
Recovery Milestones#
Each milestone represents healing and a step toward full recovery:
First time standing after surgery. Just getting upright is an accomplishment after major surgery.
First shower. Being clean after days of sponge baths feels remarkably restorative.
Drains removed. One less thing attached to your body, one less thing to manage.
Catheter removed. Urinating on your own again feels like reclaiming autonomy. This milestone sounds simple but feels enormous after days of catheter management.
First walk outside. Fresh air and sunlight after days indoors marks returning to the world.
First successful dilation session (for vaginoplasty patients). Getting through what can be a daunting new routine is worth acknowledging.
Each week completed. You're one week further into healing. That counts.
First meal with real appetite. When food sounds good again, your body is stabilizing.
First bowel movement (especially significant given constipation struggles).
First time laughing without pain.
First time looking at your surgical site and feeling something positive.
First good night's sleep.
First day you forget to take pain medication because you didn't need it.
First drive, first day back at work, first social outing. Each "first" represents reclaiming normal life.
Permission to Celebrate#
Acknowledge progress. Say out loud, "I did that." Tell your caregiver. Post about it in community spaces if you're comfortable doing so. Progress matters.
Make milestones special in whatever way resonates with you. Maybe that's a video call with friends, a favorite food, a small purchase, or just a moment of quiet gratitude. Marking progress helps you see how far you've come when the remaining distance feels endless.
Celebrating isn't frivolous—it sustains the morale that helps you continue doing the hard work of recovery.
Long-Term Recovery Perspective#
Healing Takes Time#
Full recovery from major surgery is measured in months to years, not days or weeks. Tissue healing, nerve regeneration, scar maturation, and psychological integration all proceed on their own timelines.
Be patient with the process. Frustration at slow progress is natural, but impatience doesn't speed healing. Your body is doing complex work that cannot be rushed.
Avoid the comparison trap. Other people's recoveries are not your recovery. Reading about someone who was back to work in three weeks when you're struggling at six weeks does not mean you're healing wrong—it means you're different people with different bodies, different procedures, and different circumstances.
Results Emerge Gradually#
Swelling obscures final results for months. What you see at one week is not what you'll see at six months; what you see at six months may not be what you'll see at two years. Early assessment of aesthetic results is unreliable.
Nerve sensation returns slowly. Full sensation development can take twelve to twenty-four months or even longer. Limited sensation early in recovery doesn't predict long-term outcomes.
Final aesthetic results typically emerge at twelve to twenty-four months post-surgery. Scars mature, swelling fully resolves, and tissues settle into their final positions over this extended period.
Psychological adjustment is ongoing. Integrating major body changes into your self-perception takes time. Euphoria, grief, adjustment, and acceptance may all cycle through as you develop a relationship with your changed body.
Your Body's Wisdom#
Trust the healing process. Your body has been repairing itself since before you were born. It knows how to heal tissue, fight infection, and restore function. Your job is to support it and stay out of its way.
Listen to your body's signals. Pain that tells you to rest is giving you useful information. Fatigue is not weakness—it's your body routing resources toward repair. When signals tell you to slow down, listen.
Rest when needed. This is not an invitation to anxiety about sufficient rest; it's permission to rest without guilt. If your body wants to sleep, let it sleep. If your body wants stillness, be still.
Advocate for yourself when something seems wrong. You know this body better than anyone. If something doesn't feel right, speak up. Persistent advocacy in the face of dismissal is sometimes necessary to get appropriate care.
You've already demonstrated remarkable tenacity by reaching this point—navigating systems, obtaining care, and enduring major surgery in pursuit of alignment between your body and your identity. That same strength serves you now, as you do the quiet, daily work of healing.

What does allowing yourself to fully rest—without productivity, without proving anything—feel like? What makes this easy or difficult for you?
Who are the people you can rely on during recovery, and what specific kinds of support are you comfortable asking each of them for?
What will you do to take care of your emotional health during recovery, especially if you experience post-operative depression or difficult feelings?
